CICARE沟通模式对子宫内膜癌子宫全切患者负性情绪及生存质量的影响

Effects of CICARE communication mode on negative emotion and quality of life in endometrial carcinoma patients undergoing total hysterectomy

目的:探讨CICARE沟通模式在子宫内膜癌患者子宫全切患者心理护理中的应用及其对患者负性情绪及生存质量的影响。方法:选取2016年1月至2017年7月南阳市中心医院110例拟行子宫全切的子宫内膜癌患者,随机分为观察组和对照组各55例,对照组患者接受妇科常规住院心理干预,观察组采用CICARE沟通模式的心理护理,采用Zeng抑郁量表(SDS)及Zeng焦虑量表(SAS)评价两组患者护理前后负性情绪变化,采用生活质量评价量表(QOL-100)评价两组患者护理前后生存质量变化。结果:两组患者护理前SDS及SAS评分比较差异均无统计学意义(均P>0.05),护理后两组患者SDS及SAS评分均降低(均P<0.05),护理后观察组SAS评分及SDS评分低于护理前;护理前两组患者QOLC-E量表各维度评分比较差异均无统计学意义(均P>0.05),护理后两组患者总体生存质量、生理、心理、独立性、社会关系及环境和精神维度评分均提高(均P<0.05),护理后观察组总体生存质量、生理、心理、独立性、社会关系及环境和精神维度评分均高于对照组(均P<0.05)。结果:CICARE沟通模式能够有效改善子宫内膜癌行子宫全切患者的心理护理质量,改善患者负性情绪,提高生存质量。

Objective To explore the application of CICARE communication mode in psychological nursing of endometrial cancer patients with total hysterectomy and its effect on negative emotions and quality of life of patients.Methods A total of 110 patients with endometrial cancer who planned to undergo total hysterectomy in this hospital were selected and randomly divided into the observation group and the control group,with 55 patients in each group.The control group received routine psychological intervention in gynecology,while the observation group received psychological nursing with CICARE communication mode.Zeng Depression Scale(SDS)and Zeng Anxiety Scale(SAS)were used to evaluate the negative emotional changes before and after nursing in the two groups,and quality of life assessment scale(QOL-100)was used to evaluate the quality of life changes before and after nursing in the two groups.Results There was no significant difference in SDS and SAS scores between the two groups before nursing(P>0.05).After nursing,the scores of SDS and SAS were decreased in both groups(P<0.05).The SAS and SDS scores of the observation group after nursing were lower than those before nursing.Before nursing,there was no statistical significance in the QOLC-E scale scores between the two groups(P>0.05).After nursing,the overall quality of life,physiological,psychological,independence,social relations,environmental and spiritual dimensions of the two groups of patients were improved(P<0.05).After nursing,the overall quality of life,physiological,psychological,independence,social relations,environmental and spiritual dimensions of the observation group were higher than those of the control group(P<0.05).Conclusions The CICARE communication mode can effectively improve the psychological nursing quality of endometrial cancer patients undergoing total hysterectomy,improve the negative emotions of patients and improve the quality of life.
