摘要
目的将Foley's管宫颈扩张与人工破膜联合应用于足月引产,观察引产效果及安全性.方法选择2018年1—12月本院收治的有引产指征且宫颈Bishop评分<6分的初产妇90例作为研究对象,根据采用的引产方法随机分为观察组、对照组1和对照组2三组,每组各30例.观察组为Foley's管宫颈扩张联合人工破膜引产,对照组1为米索前列醇联合缩宫素引产,对照组2为Foley's管宫颈扩张联合缩宫素引产.对照组1及对照组2为近年来临床常用方法.观察三组促宫颈成熟的效果、诱发规律宫缩时间、第-产程时间、总产程时间、胎儿宫内窘迫发生率、阴道分娩率及产后出血量进行比较.结果观察组与对照组1在以上七个方面比较:无明显差异.观察组与对照组2比较:观察组诱发临产时间、第-产程时间、总产程时间均低于对照组2,差异有统计学意义(P<0.05);阴道分娩率高于对照组2;产后出血量、胎儿窘迫无明显差异,无统计学意义(P>0.05).结论Foley's管宫颈扩张联合人工破膜的引产方法在提高阴道分娩率中占据明显优势,降低剖宫产率.
Objective Foley's cervical dilatation combined with artificial rupture of membrane was applied to term labor induction to observe the effect and safety of labor induction.Methods 90 primiparasthose admitted to our hospital from January 2018 to December 2018 and with indications of labor induction and cervical Bishop scoreless than 6 were selected as study subjects.According to the labor induction method adopted,they were divided into observation group(30 cases),control group 1(30 cases)and control group 2(30 cases).Foley's cervical dilatation combined with artificial rupture of membrane was applied to induce labor in the observation group.Misoprostol combined with oxytocin was used to induce labor in control group 1.Foley's cervical dilatation combined with oxytocin induction was applied in control group 2.The effects of promoting cervical maturation,contractions time,first labor time,total labor time,fetal distress incidence,vaginal delivery rate and postpartum blood loss were compared among the three groups.Results There were no significant differences in the above seven aspects between observation group and control group 1.Comparison between observation group and control group 2:induced labor time,first stage labor time and total labor time in the observation group were all lower than those in the control group 2,and the differences were statistically significant(P<0.05).The rate of vaginal delivery was higher than that of control group 2.There was no significant difference in postpartum blood loss and fetal distress(P>0.05).Conclusions Foley's cervical dilatation combined with artificial rupture of membrane is of obvious advantage in improving the rate of vaginal delivery and reducing the rate of cesarean section.
