健脾益肾汤联合小剂量雷公藤多苷、厄贝沙坦对肾小球肾炎患者肾功能、炎性因子和血管内皮生长因子的影响

Effect of Jianpi Yishen Decoction Combined with Low-Dose Tripterygium Wilfordii Polyglycoside and Irbesartan on Renal Function,Inflammatory Factors and Vascular Endothelial Growth Factor in Patients with Glomerulonephritis

目的探讨健脾益肾汤联合小剂量雷公藤多苷、厄贝沙坦对肾小球肾炎患者肾功能、炎性因子和血管内皮生长因子(vascular endothelial growth factor,VEGF)的影响。方法选取肾小球肾炎86例作为研究对象,根据治疗方法不同将其分为观察组和对照组两组各43例。观察组采用健脾益肾汤联合小剂量雷公藤多苷、厄贝沙坦治疗,对照组采用小剂量雷公藤多苷联合厄贝沙坦治疗。观察比较两组治疗前后肾功能指标和血清炎性因子指标、VEGF,治疗后临床效果,以及治疗期间不良反应发生情况。结果治疗前,两组肾功能指标和血清炎性因子指标、VEGF比较差异均无统计学意义(P>0.05)。治疗后,肾功能指标和血清炎性因子指标、VEGF两组均较治疗前降低,且观察组均低于对照组,差异有统计学意义(P<0.05或P<0.01)。治疗后,观察组总有效率为95.35%高于对照组总有效率86.05%,差异有统计学意义(P<0.05)。治疗期间,观察组出现恶心1例,乏力2例;对照组出现恶心2例,乏力、瘙痒各1例。两组均未发生与本研究药物有关的严重不良反应。结论健脾益肾汤联合小剂量雷公藤多苷、厄贝沙坦治疗肾小球肾炎患者能够改善肾功能,抑制炎症反应,促进血管再生,提高治疗效果,且安全性较好。

Objective To investigate the effect of Jianpi Yishen Decoction combined with low-dose Tripterygium Wilfordii Polyglycoside and Irbesartan on renal function,inflammatory factors and vascular endothelial growth factor(VEGF)in patients with glomerulonephritis.Methods A total of 86 patients with glomerulonephritis were divided into observation group(n=43)and control group(n=43)according to different treatment methods.The control group was treated with low-dose Tripterygium Wilfordii Polyglycoside and Irbesartan,while the observation group were treated by Jianpi Yishen Decoction combined with low-dose Tripterygium Wilfordii Polyglycoside and Irbesartan.Renal function indexes,inflammatory factor indexes and VEGF were compared between the two groups before and after treatment.The curative effect was evaluated after treatment,and the incidence of adverse reactions during the treatment was recorded.Results There were no significant differences in renal function indexes,inflammatory factor indexes and VEGF between the two groups before treatment(P>0.05).Renal function indexes,inflammatory factor indexes and VEGF in the two groups after treatment were lower than those before treatment,and related indexes in the observation group were lower than those in the control group(P<0.05 or P<0.01).The total effective rate of the observation group was 95.35%after treatment,which was significantly higher than 86.05%of the control group(P<0.05).During the treatment period,the observation group had 1 case of nausea and 2 cases of fatigue;the control group had 2 cases of nausea,1 case of fatigue,and 1 case of itching.There were no serious adverse reactions related to drugs used in this study during the treatment in either group.Conclusion In the treatment of patients with glomerulonephritis,Jianpi Yishen Decoction combined with low-dose Tripterygium Wilfordii Polyglycoside and Irbesartan can further improve the renal function and blood vessel regeneration of patients with glomerulonephritis,reduce the inflammatory reaction,and improve the therapeutic effect with high safety.
