经伤椎椎弓根置钉后路内固定治疗胸腰椎骨折的疗效分析 被引量:2

Analysis of curative effect of posterior internal fixation with ppedicle bone grafting and screw fixation in treatment of thoracolumbar fractures

摘要 目的探讨经伤椎椎弓根置钉后路内固定治疗胸腰椎骨折的临床疗效。方法选取商丘市第一人民医院2016年6月—2019年6月接诊的胸腰椎骨折患者87例,随机分为对照组43例、观察组44例。对照组患者采用后路短节段椎弓根钉内固定不置螺钉,观察组患者采用经伤椎椎弓根置钉后路内固定。比较两组手术参数、临床疗效、椎体解剖结构、神经功能情况。结果观察组患者负重时间、骨折愈合时间均明显低于对照组(P<0.05);观察组治疗优良率95.45%高于对照组79.07%(P<0.05);术后3个月、术后6个月观察组前缘高度大于对照组(P<0.05),Cobb角小于对照组(P<0.05);术后6个月观察组患者神经功能优于对照组(P<0.05)。结论经伤椎椎弓根置钉后路内固定治疗胸腰椎骨折疗效确切,可有效促进患者骨折愈合,矫正受损椎体解剖结构,恢复患者脊髓神经功能。 Objective To investigate the clinical effect of posterior internal fixation with pedicle bone grafting and screw fixation in the treatment of thoracolumbar fractures.Methods Totally 87 patients with thoracolumbar fractures in our hospital from June 2016 to June 2019 were randomly divided into control group(43 cases) and observation group(44 cases). Patients in the control group were treated with posterior short segment pedicle screw fixation without screw placement, while patients in the observation group were treated with posterior internal fixation via pedicle screw fixation. Compare the two groups surgical parameters, clinical efficacy, vertebral anatomical structure and neurological function.Results The weight-bearing time and fracture healing time of the observation group were significantly lower than those of the control group(P<0.05);At 3 months and 6 months after operation, the anterior height of observation group was higher than that of control group(P<0.05), and Cobb angle was smaller than that of control group(P<0.05);6 months after operation, the neurological function of the observation group was better than that of the control group(P<0.05).Conclusion It can effectively promote the fracture healing, correct the anatomical structure of the injured vertebral body and restore the spinal nerve function of the patients.
