背景音乐联合正性暗示对重症肺炎患者纤维支气管镜灌洗吸痰过程中应激反应的影响

Effect of background music combined with positive suggestion on stress response during bronchoscopy and sputum aspiration in patients with severe pneumonia

目的探讨背景音乐联合正性暗示对重症肺炎患者纤维支气管镜灌洗吸痰过程中应激反应的影响.方法将400例重症肺炎患者依据建档顺序分为实验组与对照组,各200例.两组均予以纤维支气管镜灌洗治疗,均给予常规护理干预,实验组在此基础上给予背景音乐联合正性暗示.干预前后采用焦虑自评量表、抑郁自评量表评定两组患者的焦虑、抑郁情绪,采用简明健康状况调查量表评定两组患者的生活质量,比较两组患者吸痰过程中血压、心率波动情况及护理满意度.结果干预后两组焦虑自评量表、抑郁自评量表评分均较干预前显著下降(P<0.01),实验组较对照组下降更显著(P<0.01);两组简明健康状况调查量表各项评分均较干预前显著升高(P<0.01),实验组较对照组升高更显著(P<0.01).吸痰过程中实验组心率及血压波动情况均显著小于对照组(P<0.01),实验组护理总满意率显著高于对照组(P<0.05).结论背景音乐联合正性暗示可有效减轻重症肺炎患者纤维支气管镜灌洗吸痰过程中的应激反应,缓解负性情绪,提高生活质量及护理满意度.

Objective To explore the effect of background music combined with positive suggestion on stress response during bronchoscopic lavage and sputum aspiration in patients with severe pneumonia.Methods 400 patients with severe pneumonia were divided into experimental group and control group according to the order of estab lishment,200 cases each.Both groups were given fiberop tic bronchoscope lavage treatment and routine nursing intervention.The experimental group was given background music combined with positive suggestion.SAS and SDS were used to assess the anxiety and depression of the two groups before and after the intervention.The quality of life of the two groups was assessed by SF-36.Blood pressure,heart rate fluctuations and nursing satisfaction were compared between the two groups during sputum aspiration.Results The scores of SAS and SDS in the two groups were significantly lower than those before the intervention(P<0.01),and the experimental group decreased more significantly than the control group(P<0.01).The scores of SF-36 in both groups were signiticantly higher than those before intervention(P<0.01),and the experimental group increased more significantly than the control group(PV0.01).The heart rate and blood pressure of the experimental group were significantly lower than those of the control group during sputum aspiration(P<0.01).The total satisfaction rate of the experimental group was significantly higher than that of the control group(P<0.05).Conclusions The combination of background music and positive sug­gestion can effectively alleviate the degree of stress reaction in patients with severe pneumonia during sputum aspiration by fiberoptic bronchoscope lavage,significantly improve patients'negative emotions and quality of life,and significantly improve nursing satis­faction.
