1起奶山羊间布病传播到人间布病疫情调查

Investigation on an outbreak of brucellosis spread from dairy goats to human

目的了解奶山羊间、人间布病疫情线索、传播途径。方法调查并描述剑川县金华镇文华村羊间、人间布病疫情发现和处置过程;采集文华村所有奶山羊养殖户家庭成员血清和所有奶山羊血清进行布病抗体检测,对实验室检查结果和流行病学调查资料进行分析。结果在6户养殖户中检出布病阳性奶山羊35只。其中包括在苏某B户的43只奶山羊中检出18只,阳性率为41.86%;另外5户113只奶山羊中检出17只,阳性率为15.14%,苏某B户与其他5户的奶山羊布病阳性率的差异有统计学意义(P<0.01)。苏某B户检出的18只布病阳性奶山羊中,15只是从曲靖购入的22只奶山羊中检出,阳性率为68.18%;3只是从原有的21只羊只中检出,阳性率为14.29%,二者抗体阳性率差异有统计学意义(P<0.01)。分2次采集文华村奶山羊养殖户以及邻近的龙门村、朱柳村家庭成员及其他人员共51人份血清,检出布病抗体阳性4份,其中3人出现布病临床症状,先后到大理州人民医院就诊,诊断为布病实验室确诊病例。结果这是1起买入来自布病重疫区患布病的奶山羊导致布病在自家和本村5户的奶山羊间传播,继而波及人间的布病疫情。

Objective To understand the epidemic clue of brucellosis among dairy goats and human and trace the transmission route.Methods To investigate and describe the process of detection and disposal of brucellosis among goats and human in Wenhua village,Jinhua town,Jianchuan county;serum samples were collected from all family members of dairy goat farmers and dairy goats in Wenhua village for brucellosis antibody detection;the results of laboratory tests and epidemiological investigation were analyzed.Results A total of 35 dairy goats with brucellosis positive were detected in 6 households.Among them,18 were detected in 43 dairy goats of SU B household,the positive rate was 41.86%,and 17 were detected in 113 goats of 5 households,the positive rate was 15.14%.There was significant difference in the positive rate of brucellosis antibody among goats from SU B household and other 5 households(P<0.01);among the 18 brucellosis positive goats detected in Su B household,15 were detected from 22 goats purchased from Qujing,with a positive rate of 68.18%,and 3 were detected from 21 original goats,the positive rate was 14.29%.The difference in the positive rate of brucellosis antibodies between the goats purchased from Qujing and the original was statistically significant(P<0.01).51 serum from dairy goat farmers in Wenhua village,family members in Longmen village and Zhuliu village nearby were collected,and 4 serum were positive for brucellosis antibody,3 of them presented with clinical symptoms of brucellosis and visited Dali Prefecture People’s Hospital one after the other and were diagnosed with laboratory-confirmed cases of brucellosis.Conclusion This is an outbreak of brucellosis of buying dairy goats suffering from brucellosis from a serious epidemic area,which caused brucellosis to spread among dairy goats in their own home and other 5 households in the village,and then spread to human beings.
