天津市某社区老年人腹部超声检查结果分析

Analysis of the abdominal ultrasound of elderly people in a community in Tianjin

目的分析天津市某社区老年人腹部超声检查结果,为公共卫生服务的完善提供参考依据,进一步发挥超声检查在社区公共卫生服务中的作用。方法对2017-2019年天津市河西区友谊路街社区65岁以上常住居民腹部肝、胆、胰、脾超声检查结果进行分析。结果3年共查体21347例,阳性14551例;共检出常见疾病8种,严重疾病3种,异常征象4种,术后改变3种,意外发现5种。其中肝胆疾病发生率高且较为稳定,但差异无统计学意义(P>0.05);胆囊切除率逐年增高,差异有统计学意义(P<0.05),且内部两两比较2017年发生率低于2018年、2019年,差异有统计学意义(P<0.01)。结论该社区中老年人腹部超声检查结果阳性率高,发病率较为稳定,其中肝胆疾病最常见。胆囊切除率逐年增高,主要病因为胆囊结石和胆囊息肉,应当引起足够重视。偶有严重疾病发现,如肝癌、肝硬化等,可作为医联体转诊的依据。在腹部超声项目中加入双肾超声检查意义重大,有助于进一步提升老年人健康管理水平。

Objective To analyze the abdominal ultrasound examination results of elderly people in a community of Tianjin from 2017 to 2019,to provide reference for the improvement of public health services and play more roles of ultrasound in community public health services.Methods The ultrasonic results of liver,gall bladder,pancreas and spleen in the abdomen of residents over 65 years old in 2017-2019 in Youyi Road Street Community of Hexi District were analyzed.Results A total of 21347 people were examined,and 14,551 were positive;8 common diseases,3 serious diseases,4 abnormal signs,3 postoperative changes and 5 unexpected findings were detected in three years.The incidence of hepatobiliary diseases was high and stable,but the difference was not statistically significant(P>0.05).The rate of cholecystectomy increased year by year,the differences were statistically significant(P<0.05),the incidence of pairwise internal comparison in 2017 was lower than that in 2018 and 2019,the differences were statistically significant(P<0.01).Conclusion The positive rate of abdominal ultrasound examination in this community elderly was high,and the incidence was relatively stable.Hepatobiliary diseases were most common.The rate of cholecystectomy was increasing year by year,with the main causes of cholecystolithiasis and gallbladder polyps,which should be paid enough attention.Occasionally,the discovery of major diseases such as liver cancer,liver cirrhosis and so on could be used as the basis for the reference of Medical Association.It was of great significance to add double kidney ultrasound in abdominal ultrasound project,which was helpful to further improve the health management level of the elderly.
