更年期女性反流性食管炎的内镜表现及临床分析

Endoscopic manifestations and clinical analysis of reflux esophagitis in menopausal women

目的探讨更年期女性反流性食管炎的内镜表现及临床症状。方法选取2016年3月-2019年4月肇庆市第一人民医院收治的更年期女性反流性食管炎患者87例纳入观察组,另选取同时期收治的男性反流性食管炎患者90例作为对照组,对两组患者的诊治情况、临床和内镜表现进行比较分析。结果观察组有28.74%(25/87)患者出现反流性食管炎的典型临床表现,误诊率为57.47%。对照组有91.11%(82/90)患者出现反流性食管炎的典型临床表现,误诊率为0.00%。两组患者内镜下表现比较,差异无统计学意义(P>0.05)。观察组总有效率为80.46%,与对照组的83.33%比较,差异无统计学意义(P>0.05)。结论更年期女性反流性食管炎的临床症状常以食管外临床症状或更年期症状为主要表现,导致误诊、漏诊的风险增加。因此对于女性更年期患者发生的各类治疗无效的综合征,应考虑反流性食管炎的可能性,及时进行检查和治疗干预,提升患者预后。

Objective To explore the endoscopic manifestations and clinical symptoms of reflux esophagitis in menopausal women.Methods From March 2016 to April 2019,87 menopausal women with reflux esophagitis in zhaoqing First People’s Hospital were selected as the observation group,and another 90 male patients with reflux esophagitis in the same period were selected as the control group.The diagnosis and treatment,clinical and endoscopic manifestations of the two groups were compared and analyzed.Results In the observation group,28.74%(25/87) patients had typical clinical manifestations of reflux esophagitis,and the misdiagnosis rate was 57.47%.In the control group,91.11%(82/90) patients had typical clinical manifestations of reflux esophagitis,and the misdiagnosis rate was 0.00%.There was no significant difference in endoscopic performance between the two groups(P>0.05).The total effective rate of the observation group was 80.46%,compared with 83.33% of the control group,the difference was not statistically significant(P>0.05).Conclusion The main clinical symptoms of menopausal women reflux esophagitis were extraesophageal symptoms or climacteric symptoms,which increased the risk of misdiagnosis and missed diagnosis.Therefore,we should consider the possibility of reflux esophagitis,timely check and treatment intervention,and improve the prognosis of patients with all kinds of treatment ineffective syndrome in female climacteric patients.
