3D/4D超声诊断技术在女性盆底常见病中的应用 被引量:1

Application of three/four-dimensional ultrasound in common female pelvic floor diseases

摘要 目的探讨三维和四维(3D/4D)超声诊断技术在女性盆底常见病诊断中的应用效果。方法选取2018年8月-2020年1月天津市武清区人民医院产后女性体检人员200例为研究对象,其中患有盆底功能障碍性疾病的78例作为研究组,无盆底功能障碍性疾病的122例作为对照组。两组研究对象均采用经会阴部3D/4D超声诊断技术测量三种状态(静息状态、盆底肌收缩状态及最大Valsalva动作状态)下的盆底结构参数,并记录膀胱逼尿肌厚度(DWT)、肛提肌的完整性、膀胱颈移动度(BND)、膀胱尿道后角角度(PUVA)、尿道旋转角度(URA)、肛提肌裂孔面积(LHA)。结果研究组BND、PUVA、URA、尿道内口漏斗形成率、LHA大于对照组,差异均有统计学意义(P<0.05),两组DWT相比差异无统计学意义(P>0.05)。结论经会阴3D/4D超声在女性盆底障碍性疾病诊断中,具有较高的应用价值,值得在临床中推广应用。 Objective To explore the application effect of three/four-dimensional(3D/4D) ultrasound diagnosis technology in the diagnosis of common female pelvic floor diseases.Methods From August 2018 to January 2020,200 cases of postpartum female physical examination personnel in Tianjin Wuqing District People’s Hospital were selected as the research objects.Among them,78 cases with pelvic floor dysfunction were chosen as the research group and 122 cases without pelvic floor dysfunction as the control group.The parameters of pelvic floor structure in three states(resting state,pelvic floor muscle contraction state and maximum Valsalva movement state) were measured by transperineal 3D/4D ultrasound technology,and the thickness of bladder detrusor(DWT),integrity of levator ani muscle,bladder neck mobility(BND),posterior vesicourethral angle(PUVA),urethral rotation angle(URA) and levator ani hiatus area(LHA) were recorded.Results BND,PUVA,URA,funnel formation rate of urethral orifice and LHA in the research group were significantly higher than those in the control group,the differences were statistically significant(P<0.05),but there was no statistically significant difference in DWT between the two groups(P>0.05).Conclusion Transperineal 3D/4D ultrasound had high application value in the diagnosis of pelvic floor dysfunction,which was worthy of clinical promotion and application.
