下肢皮肤Merkel细胞癌一例

A case of Merkel cell carcinoma on the lower limb

72岁女性患者,左胫前无痛性暗红色结节渐增大2年。皮肤科情况:左胫前暗红色半球形隆起性质软结节,表面较多鳞屑,基底浸润感。组织病理示:表皮萎缩变薄,真皮内可见肿瘤细胞团,瘤细胞呈条索状及腺样排列,异形性明显,细胞周围有黏液样物质沉积。诊断:皮肤Merkel细胞癌。给予局部扩大切除,术后恢复良好,随访1年余无复发。

A 72-year-old female patient presented with a gradually enlarged painless dark red nodule in front of the left tibia for 2 years.Physical examination showed that a dark red hemispherical protuberant soft nodule located in front of the left tibia with many scales on the surface and basal infiltration.Histopathological examination findings:epidermis atrophied and thinned,the tumor masses composed of cells arranged in cord-like and adenoid manner distributed in the dermis with obvious heteromorphism,as well as mucoid substances deposited around the cells.A diagnosis of Merkel cell carcinoma was made.Local enlarged resection was given and the patient was followed up for more than one year without recurrence.
