2型糖尿病住院患者背俞穴反应点分布规律研究

Study on the regularity of back-shu point reaction point in type 2 diabetes mellitus inpatients

目的:从经络切诊角度,探讨2型糖尿病(T2DM)住院患者背俞穴反应点分布规律。方法:纳入T2DM住院患者82例,收集切诊当日的随机血糖、背俞穴阳性反应点资料,进行统计学分析。结果:T2DM住院患者的高频背俞穴阳性反应点分别为胃俞(32.9%)、肝俞(29.3%)、肾俞(26.8%)、胰俞(19.5%)、三焦俞(18.3%);阳性反应表现占比为结节(45.1%)、条索(26.1%)、僵紧(16.2%)、下陷(4.9%)、冷(3.5%)等;肾俞阳性反应与血糖呈正相关(P<0.05)。结论:T2DM住院患者背俞穴阳性反应点分布规律,提示T2DM住院患者可从脾胃、肝肾论治,配以胰俞等穴;背俞穴出现结节,应辨证湿浊或痰湿,以明确治疗;出现僵紧、冷或条索,可考虑体内存在寒气;出现下陷,应重补肾;出现血糖异常时,可选肾俞穴治疗。

Objective:To explore the distribution of reaction points of Back-shu points in patients with type 2 diabetes mellitus(T2 DM)from prospective of the meridian palpation and pulse taking.Methods:A total of 82 inpatients with T2 DM were included in the study.The data of blood glucose and positive reaction points of Back-shu points were collected and analyzed statistically.Results:The frequency of the positive reaction points of Back-shu points in T2 DM inpatients were Weishu(BL 21)(32.9%),Ganshu(BL 18)(29.3%),Shenshu(BL 23)(26.8%),Yishu(EX-B5)(19.5%),Sanjiaoshu(BL 22)(18.3%).The proportion of the positive reaction points was nodule(45.1%),cord(26.1%),rigidity(16.2%),subsidence(4.9%),cold(3.5%),etc..The positive reaction of Shenshu(BL 23)was positively correlated with blood glucose(P<0.05).Conclusion:The distribution law of the positive reaction points of back-shu points in T2 DM patients suggests that the T2 DM inpatients can be treated from spleen and stomach,liver and kidney with Yishu(EX-B5)and other acupoints;the nodules of back-shu points should be treated by syndrome differentiation of dampness or phlegm dampness,so as to make clear the treatment;the cold in the body can be considered in case of rigidity,cold or cord;the sinking should be treated by kidney replenishment;in case of abnormal blood sugar,Shenshu(BL 23)can be selected for treatment.
