家庭医生指导呼吸训练对老年COPD患者肺功能的影响

Family physicians guide the effects of respiratory training on lung function in elderly patients with COPD

目的探讨基于家庭医生指导的呼吸训练对老年慢性阻塞性肺疾病(COPD)患者肺功能的影响。方法选取2018年1月—2019年8月东莞市沙田镇社区卫生服务中心确诊为COPD的200例老年患者作为研究对象,将自愿签约家庭医生的101例患者作为观察组,其余99例未签约家庭医生的患者作为对照组。对照组按COPD患者健康管理服务规范要求给予服务,观察组在对照组的基础上实施家庭医生指导呼吸训练。比较2组患者干预前、后肺功能指标及呼吸困难程度。结果干预前2组肺功能各项指标比较均无统计学差异(P>0.05);干预后2组指标均有所升高,且观察组各项指标优于对照组,差异有统计学意义(P<0.05)。观察组患者干预后呼吸困难程度低于对照组,差异有统计学意义(P<0.05)。结论家庭医生指导的呼吸训练对缓解老年COPD患者的呼吸困难和改善肺功能有明显的促进作用。

Objective To investigate the effect of respiratory training guided by family doctor on pulmonary function in elderly patients with COPD.Methods 200 elderly patients with chronic obstructive pulmonary disease(COPD)confirmed by The Community Health Service Center of Shatian Town,Dongguan City from January 2018 to August 2019 were selected as the research objects.A total of 101 patients voluntarily signed up as a family doctor were used as the observation group,and the remaining 99 patients who were not signed up as the control group.The control group was provided with services according to the health management service specifications for patients with chronic obstructive pulmonary disease(COPD),and the observation group was given respiratory training guided by a family doctor on the basis of the control group.The pulmonary function indexes and the degree of dyspnea before and after intervention were compared between two groups.Results Before the intervention,there was no significant difference in lung function between the two groups(P>0.05);after the intervention,the indexes of both groups were increased(P<0.05),and the indexes in the observation group were superior to those in the control group,with statistically significant differences(P<0.05);the degree of dyspnea in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Respiratory training guided by family doctors can alleviate dyspnea and improve lung function in elderly patients with COPD.
