ACR TI-RADS联合超微血流成像技术对甲状腺良恶性结节的临床鉴别诊断分析 被引量:1

摘要 目的研究ACR TI-RADS联合超微血流成像(SMI)技术对甲状腺良恶性结节的临床鉴别诊断价值。方法选择2019年7月—2020年7月惠东县人民医院收治的90例甲状腺结节患者作为研究对象,使用CDFI、SMI对患者血流分布情况及血供特征进行检测,以病理学检查结果为金标准,比较ACR TI-RADS与SMI技术的诊断价值。结果CDFI、SMI对患者血流分级的检出率分别为66.67%,80.39%,SMI技术检出率高于CDFI;恶性结节的Ⅳ型、Ⅴ型分布比例高于良性结节,Ⅰ型、Ⅱ型分布比例低于良性结节;SMI技术联合ACR TI-RADS诊断的敏感度、准确度及特异度均高于单独使用ACR TI-RADS,且SMI技术联合ACR TI-RADS的敏感度、准确度均高于CDFI技术联合ACR TI-RADS(P<0.05)。结论在临床诊断甲状腺良恶性结节中,相比于SMI,ACR TI-RADS联合SMI可显著提高血流检出率,且可提高诊断的准确度、敏感度及特异度,对良恶性甲状腺的诊断有一定鉴别价值。 Objective To study the clinical differential diagnosis value of ACR TI-RADS combined with SMI technology for benign and malignant thyroid nodules.Methods Ninety patients with thyroid nodules admitted to the Hospital between July 2019 and July 2020 were enrolled.All patients were given thyroid examination with pathological examination results as the gold standard,and CDFI and SMI were used to determine the blood flow distribution and blood supply characteristics of the patients.The diagnostic value of ACR TI-RADS and SMI technology was compared through analysis.Results The detection rates of CDFI and SMI for blood flow classification were 66.67%and 80.39%,respectively,where the detection rate of SMI was higher than that of CDFI.The distribution ratio of type IV and type V malignant nodules was higher than that of benign nodules,and the distribution ratio of type I and type II was lower than that of benign nodules.The sensitivity,accuracy and specificity of the diagnosis via SMI technology combined with ACR TI-RADS were higher than those diagnosed with ACR TI-RADS alone,and the sensitivity and accuracy of SMI technology combined with ACR TI-RADS were higher than those of CDFI technology combined with ACR TI-RADS(all P<0.05).Conclusion For the clinical diagnosis of benign and malignant thyroid nodules,compared with SMI,ACR TI-RADS combined with SMI can significantly increase the detection rate of blood flow,and can improve the accuracy,sensitivity and specificity of diagnosis;therefore,it is of certain differential value for the diagnosis of benign and malignant thyroid nodules.
