潜在不适当用药增加门诊老年患者住院风险的真实世界研究

Potentially Inappropriate Medications Increasing the Risk of Hospital Admission in Elderly Outpatients:A Real-World Research

目的分析门诊老年患者潜在不适当用药(PIM)的使用情况,为临床合理用药提供参考。方法采用真实世界注册登记法,对门诊老年(≥65岁)患者进行注册登记;依据中国老年人PIM标准筛查患者处方,并对患者性别、年龄、PIM使用情况、住院信息等进行统计与分析。结果纳入患者1002例中,使用PIM 203例(20.26%)、215例次,涉及14种2类PIM;使用PIM与未使用PIM患者的年龄、性别等无显著差异(P>0.05);使用PIM患者60 d内住院率为11.82%,显著高于未使用PIM患者的6.88%(P<0.05)。结论门诊老年患者住院风险增加可能与使用PIM有关,需加强相应管理,提高用药安全。

Objective To analyze the usage of potentially inappropriate medications(PIM)in elderly outpatients,and to provide a reference for rational use of drugs in the clinic.Methods The elderly(≥65 years old)outpatients were registered by the registry study in the real-world research.The prescriptions of outpatients were screened by the Criteria of Potentially Inappropriate Medications for Older Adults in China,and the information of outpatients’gender,age,usage of PIM and hospitalization costs was analyzed.Results Totally 1002 elderly outpatients(≥65 years old)met the criteria,and 203 patients(20.26%)used PIM,which included 215 prescriptions,14 kinds and two types of PIM.There was no significant difference in age and gender between the patients with and without PIM(P>0.05).Precisely 11.82%of outpatients who used PIM received hospital admission within 60 d,which was significantly higher than 6.88%of the outpatients who did not use to PIM(P<0.05).Conclusion The usage of PIM in elderly outpatients may increase the risk of hospital admission,and the corresponding management should be strengthened to improve the medication safety.
