复方倍他米松局部注射联合关节松动术治疗腕管综合征临床研究

Clinical Study of Local Injection of Compound Betamethasone Combined with Joint Mobilization in the Treatment of Patients with Carpal Tunnel Syndrome

目的探讨复方倍他米松局部注射联合关节松动术治疗腕管综合征的临床疗效,以及对腕管压力指标的影响。方法选取河池市宜州区人民医院及柳州钢铁集团有限公司医院2017年9月至2019年9月收治的腕管综合征患者94例,随机分为观察组和对照组,各47例。两组患者均予关节松动术,观察组患者加用复方倍他米松注射液局部注射治疗,均连续治疗8周。结果观察组总有效率为89.36%,显著高于对照组的72.34%(P<0.05);治疗后,观察组患者的休息位、握拳位腕管压力及C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)水平均显著低于对照组(P<0.05);观察组与对照组不良反应发生率相当(17.02%比10.64%,P=0.370>0.05)。结论复方倍他米松局部注射联合关节松动术治疗腕管综合征,能降低腕管压力,改善血清学指标水平。

Objective To investigate the clinical efficacy of local injection of compound betamethasone combined with joint mobilization in the treatment of patients with carpal tunnel syndrome and its effect on the pressure index of carpal tunnel.Methods Totally 94 patients with carpal tunnel syndrome admitted to The People’s Hospital of Yizhou from September 2017 to September 2019 were selected and randomly divided into the observation group and the control group,47 cases in each group.The patients in the two groups were treated with joint mobilization,and the patients in the two groups were given local injection of Compound Betamethasone Injection.Both groups were continuously treated for eight weeks.Results The total effective rate of the observation group was 89.36%,which was significantly higher than 72.34%of the control group(P<0.05).After treatment,the carpal tunnel pressure at rest position and clenching position,the levels of C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(17.02%vs.10.64%,P=0.370>0.05).Conclusion Local injection of compound betamethasone combined with joint mobilization in the treatment of patients with carpal tunnel syndrome can reduce the carpal tunnel pressure and improve the levels of serological indexes.
