微创经椎间孔腰椎椎间融合术治疗腰椎间盘突出症患者的效果

Effects of minimally invasive transforaminal lumbar interbody fusion in treatment of patients with lumbar disc herniation

目的:观察微创经椎间孔腰椎椎间融合术治疗腰椎间盘突出症(LDH)患者的效果。方法:选取72例LDH患者作为研究对象,按照随机数字表法分为对照组与观察组各36例。对照组选用传统手术治疗,观察组选用微创经椎间孔腰椎椎间融合术治疗。比较两组手术相关指标水平、自理能力评分、视觉模拟评分法(VAS)评分、腰椎功能评分、治疗效果和并发症发生率。结果:观察组手术时间、卧床时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后7 d,两组VAS评分均低于术前,且观察组低于对照组,两组日本骨科学会腰痛疾患评定表评分和日常生活能力评定量表评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率为97.22%,高于对照组的77.78%,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:微创经椎间孔腰椎椎间融合术治疗LDH患者可缩短手术时间、卧床时间和住院时间,减少术中出血量,降低VAS评分和并发症发生率,提高治疗总有效率和腰椎功能评分,优于传统手术治疗效果。

Objective:To study effects of minimally invasive transforaminal lumbar interbody fusion in treatment of patients with lumbar disc herniation(LDH).Methods:72 LDH patients were selected as the research objects,and were divided into control group and observation group according to the random number table method,36 cases in each group.The control group was treated with traditional surgical treatment,while the observation group was treated with minimally invasive transforaminal lumbar interbody fusion.The levels of surgery-related indicators,visual analogue scale(VAS)scores before and 7 days after the surgery,lumbar function score,treatment effects,and complication rate were compared between the two groups.Results:The operation time,bed time and hospitalization time of the observation group were shorter than those of the control group;the intraoperative blood loss was less than that of the control group;and the differences were statistically significant(P<0.05).7 days after the surgery,the VAS scores of the two groups were lower than those before the surgery,and that of the observation group was lower than that of the control group;the JOA scores and ADL scores of the two groups were higher than those before the surgery,and those of the observation group were significantly lower than those of the control group;and the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 97.22%,which was higher than 77.78% in the control group,and the difference was statistically significant(P<0.05).Further,the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Minimally invasive transforaminal lumbar interbody fusion in the treatment of the patients with LDH can shorten the operation time,bed time and hospitalization time,reduce the intraoperative blood loss,reduce the VAS score and complication rate,and improve the total treatment effective rate and lumbar function score.Moreover,it is superior to traditional surgical treatment.
