血清胱抑素C与N末端脑钠肽前体对高血压心力衰竭患者预后的评估价值

Value of serum cystatin C and N-terminal pro-brain natriuretic peptide in evaluation of prognosis of patients with hypertensive heart failure

目的:探讨血清胱抑素C(CysC)与N末端脑钠肽前体(NT-proBNP)对高血压心力衰竭患者预后的评估价值。方法:回顾性分析2019年1月至2020年6月115例高血压患者的临床资料,依据是否发生心力衰竭分为高血压组(n=55)和高血压心力衰竭组(n=60),另选取同期行常规体检的60名健康者作为对照组。比较三组及不同心功能分级、预后的高血压心力衰竭患者的CysC与NT-proBNP水平。结果:高血压心力衰竭组CysC、NT-proBNP水平均高于高血压组和对照组,差异有统计学意义(P<0.05);随着心功能分级的增加,高血压心力衰竭患者CysC、NT-proBNP水平均不断升高,差异有统计学意义(P<0.05);随访6个月,高血压心力衰竭死亡患者CysC、NT-proBNP水平均高于存活患者,差异有统计学意义(P<0.05)。结论:CysC与NT-proBNP水平检测在高血压心力衰竭患者预后的评估中具有重要的临床应用价值。

Objective:To investigate value of serum cystatin C(CysC)and N-terminal pro-brain natriuretic peptide(NT-proBNP)in evaluation of prognosis of patients with hypertensive heart failure.Methods:The clinical data of 115 patients with hypertension from January 2019 to June 2020 were retrospectively analyzed.According to whether heart failure occurred,they were divided into hypertension group(n=55)and hypertensive heart failure group(n=60).In addition,60 healthy individuals who underwent routine physical examination during the same period were selected as the control group.The cardiac functional grading,the prognosis and the levels of CysC and NT-proBNP were compared among the three groups.Results:The levels of CysC and NT-proBNP in the hypertensive heart failure group were higher than those in the hypertension group and the control group,and the differences were statistically significant(P<0.05).With the increase of the cardiac functional grading,the levels of CysC and NT-proBNP in the patients with hypertensive heart failure continued to increase,and the differences were statistically significant(P<0.05).Among the follow-up of 6 months,the levels of CysC and NT-proBNP in the patients who died of hypertensive heart failure were higher than those of the surviving patients,and the differences were statistically significant(P<0.05).Conclusions:The detection of CysC and NT-proBNP levels has important clinical application value in the evaluation of the prognosis of the patients with hypertensive heart failure.
