布地奈德混悬液联合多索茶碱对慢性阻塞性肺疾病患者疗效、肺功能及血气分析的影响 被引量:13

摘要 目的分析布地奈德混悬液联合多索茶碱对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者疗效、肺功能及血气分析的影响。方法选取2016年3月—2020年1月我院收治的COPD患者104例,根据治疗方法的不同分为研究组和对照组各52例。2组入院后予止咳、化痰、抗炎、补液、营养支持、机械通气、防治并发症等对症支持治疗,在此基础上对照组加用多索茶碱葡萄糖注射液缓慢静脉滴注,研究组在对照组基础上予吸入用布地奈德混悬液治疗,2组均治疗2周。比较2组临床疗效、治疗前后肺功能及血气分析指标变化情况和治疗后不良反应发生率。结果治疗后研究组总有效率高于对照组(P<0.05)。治疗后,2组最大自主通气量(MVV)、第一秒用力呼气容积占预计值的百分比(FEV1%)、呼气中断流速(MMEF)均明显高于治疗前,且研究组MVV、FEV1%、MMEF亦高于对照组,差异有统计学意义(P<0.05或P<0.01)。与治疗前相比,2组动脉血pH、氧分压(PaO 2)均有升高,动脉血二氧化碳分压(PaCO 2)均下降,差异有统计学意义(P<0.05);治疗后,研究组动脉血pH、PaO 2升高幅度和PaCO 2下降幅度明显大于对照组(P<0.01)。2组总不良反应发生率比较差异无统计学意义(P>0.05)。结论布地奈德混悬液联合多索茶碱能提高COPD患者临床疗效,改善患者肺功能,提高动脉血pH、PaO 2,降低PaCO 2,且耐受性好、安全性高。 Objective To analyze the effect of budesonide suspension combined with doxofylline on the efficacy,pulmonary function and blood gas analysis of patients with chronic obstructive pulmonary disease(COPD).Methods A total of 104 patients with COPD treated in our hospital from March 2016 to January 2020 were selected and divided into research group and control group according to different treatment methods,with 52 cases in each group.After admission,the two groups were given antitussive,expectorant,and anti-inflammatory treatment,rehydration,nutritional support,mechanical ventilation,prevention and treatment of complications and other symptomatic support treatment.On this basis,the control group was given doxofylline glucose injection by slow intravenous drip,and the research group was given budesonide suspension inhalation on the basis of the control group.Both groups were treated for two weeks.The clinical efficacy,changes of pulmonary function and blood gas analysis indicators before and after treatment,and the incidence of adverse reactions after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group(P<0.05).After treatment,MVV,FEV1%and MMEF of the two groups were significantly higher than those before treatment,and MVV,FEV1%and MMEF of the research group were also higher than those of the control group(P<0.05 or P<0.01).Compared with those before treatment,the arterial blood pH and PaO 2 of the two groups were increased,and the arterial blood carbon dioxide partial pressure(PaCO 2)was decreased(P<0.05).After treatment,the increase of arterial blood pH,PaO 2 and the decrease of PaCO 2 of the research group were significantly greater than those of the control group(P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Budesonide suspension combined with doxofylline can improve the clinical efficacy of COPD patients,improve pulmonary function,increase arterial blood pH,PaO 2,and reduce PaCO 2,with good tolerance and high safety.
