五位一体疗法治疗腰椎间盘突出症疗效观察

Curative Observation of Applying Five-in-one Therapy in Treatting Lumbar Disc Herniation

目的:评价五位一体疗法治疗腰椎间盘突出(LHD)的有效性及安全性。方法:选取山东中医药大学第二附属医院门诊及病房自2017年8月~2019年5月就诊的LHD患者120例,随机分成三组,每组40例,常规组给予口服腰痛宁胶囊治疗,挑络组给予口服腰痛宁胶囊、挑络治疗,五位一体组给予口服腰痛宁胶囊与五位一体疗法(包括挑络、拔罐、外用药物、红外线理疗、穴位贴敷)治疗,1周为1个疗程,观察比较三组患者治疗前后的症状改善及临床疗效、视觉模拟评分法(VAS)、日本骨科协会腰椎功能评分量表(JOA)、挑络疼痛量表,并观察有无不良反应。结果:五位一体疗法组临床疗效总有效率为97.5%,挑络组总有效率为92.5%,常规组总有效率为85%,五位一体疗法组JOA评分、VAS评分、挑络疼痛量表评分均优于另外两组(P<0.05),挑络组JOA评分、VAS评分、挑络疼痛量表评分优于常规组(P<0.05),未发生明显不良反应。结论:挑络疗法作为中医特色疗法在治疗LHD时有效安全,五位一体疗法较之疗效更加显著,作用更加全面,本研究为临床治疗LHD提供了新的方法,也为中医特色疗法应用于临床提供了新的思路,具有重要的临床研究价值。

Objective:To evaluate the efficacy and safety of Five-in-one Therapy for lumbar disc herniation(LHD).Methods:120cases of LHD patients were randomly divided into three groups,40cases in each group.The routine group was treated with oral Yaotongning Capsule,the selective group was treated with oral Yaotongning Capsule and flour-picking therapy,and the five-in-one group was treated with oral Yaotongning Capsule and the Five-in-one Therapy(including flour-picking,cupping,external medicine,infrared physical therapy,acupuncture collaterals)treatment,a course of treatment for one week,observing and comparing the three groups of patients before and after treatment of symptoms improvement and clinical efficacy,visual analogue scoring(VAS),Japanese Orthopedic Association lumbar function scoring scale(JOA),pick up pain scale,and observing whether there were adverse reactions.Results:The total effective rate was 97.5%in the five-in-one therapy group,92.5%in the selective group and 85%in the conventional group.The JOA score,VAS score and the selective pain scale score in the fivein-one therapy group were better than those in the other two groups(P<0.05).The JOA score,VAS score and the selective pain scale score in the selective group were better than those in the conventional group(P<0.05),and no significant adverse reactions occurred.Conclusion:As a characteristic therapy of traditional Chinese medicine,acupuncture collaterals therapy plays a significant role in the treatment of lumbar disc herniation.The Five-in-one Therapy is more effective and comprehensive.This study provides a new method for the clinical treatment of LHD,and also provides a new idea for the application of traditional Chinese medicine characteristic therapy in clinical,which has important clinical research value.
