椎体成形术后再骨折相关因素分析

Analysis of Related Factors of Re-fracture after Vertebroplasty

目的:探究椎体成形术后再骨折的相关因素.方法:选取在2017年1月-2019年12月我院收治的97例骨质疏松性椎体压缩性骨折患者,所有患者均进行椎体成形术治疗.其中有27例出现再发骨折情况(观察组),70例没有出现再发骨折情况(对照组).对所有患者的临床病历资料进行回顾性分析,并对患者的性别、年龄、终末随访定量CT值、空腔样椎体数目、手术前后VAS评分等相关情况进行准确记录.结果:在性别、年龄、空腔样椎体数目等相关方面,2组差异无统计学意义(P<0.05);观察组终末随访定量CT值mg/cc,低于对照组终末随访定量CT值(69.24±12.35)mg/cc,差异具备统计学意义(P<0.05);与手术前相比,2组术后第1天VAS评分与其差异具备统计学意义(P<0.05),术后1天和末次随访相比两者存在统计学意义(P<0.05),末次随访时2组存在统计学意义(P<0.05).结论:运用椎体成形术治疗骨质疏松性椎体压缩性骨折患者能够获得良好的治疗效果,但是再发骨折的发生和患者的性别、年龄、空腔样椎体数目等因素之间不存在相关性,导致椎体成形术后再骨折发生的原因可能为骨质疏松.

Objective:To explore the related factors of refracture after vertebroplasty.Methods:From January 2017 to December 2019,97 patients with osteoporotic vertebral compression fracture were selected and treated with vertebroplasty.Among them,there were 27 cases with recurrent fractures(observation group),70 cases without recurrent fractures(control group).The clinical data of all patients were analyzed retrospectively,and the patients'gender,age,quantitative CT value at the end of follow-up,number of cavity like vertebrae,VAS score before and after operation were recorded accurately.Results:There was no significant difference between the two groups in terms of sex,age and the number of cavity like vertebrae(P<0.05);the final follow-up quantitative CT value mg/CC in the observation group was lower than that in the control group(69.24±12.35)mg/cc,the difference was statistically significant(P<0.05);compared with that before operation,the VAS score of the first day after operation in the two groups was statistically significant(P<0.05),There was significant difference between the two groups in the last follow-up(P<0.05).Conclusion:The treatment of osteoporotic vertebral compression fractures with vertebroplasty can achieve good results,but there is no correlation between the incidence of recurrent fractures and the factors such as gender,age,number of hollow vertebrae,etc.,which may lead to the occurrence of re fractures after vertebroplasty.
