平板运动和动态心电图对无症状心肌缺血的诊断价值

Diagnostic Value of Treadmill Exercise and Holter in Silent Myocardial Ischemia

目的:探讨平板运动试验(TET)和动态心电图(DECG)对无症状心肌缺血的诊断价值。方法:选择2018年1月~2020年12月收治的疑似无症状心肌缺血的患者120例作为研究对象,最后确诊均以冠状动脉造影(CAG)为诊断标准,比较TET、DECG对无症状心肌缺血的诊断价值。结果:动态心电图检查对无症状心肌缺血阳性检测率为73.97%,阴性检测率为42.55%,诊断精准度为66.67%,敏感度为66.67%,特异性为51.28%;平板运动试验对无症状心肌缺血的阳性检测率为90.67%,阴性检测率为71.11%,诊断精准度为83.33%,敏感度为83.95%,特异性为82.05%。平板运动试验以上各指标均明显超过动态心电图检查(P<0.05)。结论:TET对无症状心肌缺血的诊断准确性较高,显著优于DECG的诊断,对无症状心肌缺血具有较高的诊断价值。

Objective:To investigate the diagnostic value of treadmill exercise test(TET)and dynamic electrocardiogram(DECG)in silent myocardial ischemia.Methods:120 patients with suspected asymptomatic myocardial ischemia admitted from January 2018 to December 2020 were selected as the research objects.The final diagnosis was based on coronary angiography(CAG).The diagnostic value of TET and DECG for asymptomatic myocardial ischemia was compared.Results:The positive detection rate of DCG for silent myocardial ischemia was 73.97%,the negative detection rate was 42.55%,the diagnostic accuracy was 66.67%,the sensitivity was 66.67%,and the specificity was 66.67%.The positive detection rate of treadmill exercise test for silent myocardial ischemia was 90.67%,the negative detection rate was 71.11%,the diagnostic accuracy was 83.33%,the sensitivity was 83.95%,and the specificity was 82.05%.The above indicators of the TET were significantly higher than the DECG examination(P<0.05).Conclusion:TET is more accurate than DECG in the diagnosis of silent myocardial ischemia,and has higher diagnostic value in silent myocardial ischemia.
