互联网医院模式下的医联体分级诊疗服务探索及初步实践

探索互联网医院模式在医联体分级诊疗服务开展中的应用。文章以上海市浦东新区人民医院及医联体内的浦东新区合庆社区卫生服务中心、浦东新区川沙社区卫生服务中心为例,通过前期开展社区需求调研,了解社区对互联网诊疗的需求程度以及患者对互联网诊疗的可操作程度,调研表明互联网医院模式下的分级诊疗服务具有可实现性;再从国家信息化建设标准和智慧医院建设要求出发,设计互联网医院模式下的医联体转诊业务情景,构建互联网医院模式下的医联体分级诊疗业务体系。互联网医院与分级诊疗的结合是一种新的探索方向,基于互联网医院模式下的医联体分级诊疗实施路径,不仅可以方便患者就医,同时也快速加强医联体内机构医师资源、医院床位资源、医院软硬件资源等共享,从而提升区域医疗机构整体医疗服务能力,加强医联体内机构协同高效的发展,缓解居民“看病难”问题。实践表明,通过互联网医院模式下的医联体分级诊疗服务的拓展,可构筑医联体内上下联动的在线诊疗服务,解决分级诊疗不连贯性和时间滞后性的难点,使医联体的医疗机构协同能力更广泛互联,满足患者基层就诊便能享受综合性医院的诊疗服务,互联网分级诊疗必将成为近阶段一个重要的医改方向。

Explore the application of the Internet hospital model in the development of medical consortium hierarchical diagnosis and treatment services.Taking the Shanghai Pudong New Area People's Hospital,and the Pudong New Area Heqing Community Health Service Centerand the Pudong New Area Chuansha Community Health Service Center of medical consortium as examples,this paper exploresthe community’s demand for Internet diagnosis and treatment and the operability of Internet diagnosis and treatment for patients through preliminary community demand surveys.The surveys show that hierarchical diagnosis and treatment services under the Internet hospital model are feasible.Starting from the national informatization construction standards and smart hospital construction requirements,this paper designs the medical consortium referral situation based on the Internet hospital model,and builds a medical consortium hierarchical diagnosis and treatment systemunder the Internet hospital model.The combination of the Internet hospital model and the hierarchical diagnosis and treatment is a new direction for exploration.The implementation path of medical consortium hierarchical diagnosis and treatment services based on the Internet hospital model quickly strengthens the sharing of doctor resources,hospital bed resources,hospital software and hardware resources of medical consortium while facilitating patients'access to medical treatment,so as to improve the overall medical service capacity of regional medical institutions,strengthen the coordinated and efficient development of medical consortium institutions,and alleviate the problem of residents'"Difficult to see a doctor when ill".Practice shows that through the expansion of medical consortium hierarchical diagnosis and treatment services under the Internet hospital model,online diagnosis and treatment services can be built with upper and lower linkage in the medical consortium to solve the difficulties of incoherence and time lag of hierarchical diagnosis and treatment,so that the collaboration capability of medical consortium institutions can be more widely interconnected,and patients can enjoy the diagnosis and treatment services of a comprehensive hospital when they go to the grassroots level.Internet hierarchical diagnosis and treatment will certainly become an important direction of medical reform in the recent stage.
