肝内胆管结石患者常规肝切除与精准肝切除的对比

Comparison of Conventional Hepatectomy and Precise Hepatectomy in Patients With Intrahepatic Bile Duct Stones

目的对比分析精准肝切除与常规切除治疗肝内胆管结石的血清学指标、肝功能及并发症情况。方法经随机数字表法将2018年2月—2020年3月收治的60例肝内胆管结石患者分成两组,对照组与试验组各30例,分别采用常规切除、精准肝切除治疗,对比两组患者的血清学指标、肝功能、并发症情况。结果两组治疗后的各项血清学指标(IL-17、IL-23、NF-κB)、各项肝功能指标(TBIL、ALP、ALT)水平均显著降低(P<0.05);而且试验组的降低幅度更加显著(P<0.05)。在并发症发生率方面,试验组明显低于对照组(P<0.05)。结论选择精准肝切除术对肝内胆管结石患者进行治疗,能让炎性反应明显减轻,对肝组织损伤进行有效控制,而且术后并发症少,具有较高的安全性,值得临床应用。

Objective To compare the serological indexes,liver function and complications between precise hepatectomy and conventional hepatectomy in the treatment of intrahepatic bile duct stones.Methods 60 patients with intrahepatic bile duct stones from February 2018 to March 2020 were randomly divided into two groups,30 cases in the control group and 30 cases in the experimental group,respectively,treated with conventional resection and precise hepatectomy,and the serological indexes,liver function and complications of the two groups were compared.Results After treatment,the levels of serum indexes(IL-17,IL-23,NF-κB)and liver function indexes(TBIL,ALP,ALT)in the two groups were significantly decreased(P<0.05),and the decrease in the experimental group was more significant(P<0.05).The incidence of complications in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion Precise hepatectomy for the treatment of patients with intrahepatic bile duct stones can significantly reduce the inflammatory reaction,effectively control the liver tissue damage,and has less postoperative complications,with high safety,which is worthy of clinical application.
