自拟清阳祛邪汤联合中药外敷治疗亚急性甲状腺炎的疗效研究

Clinical Observation Self-made Qingyang Quxie Decoction Combined with Topical Application of TCM for Subacute Thyroiditis

目的通过观察自拟清阳祛邪汤联合中药外敷治疗亚急性甲状腺炎的疗效,探讨中医药治疗亚急性甲状腺炎的优势。方法患者66例按照随机数字法分为中药治疗组与西药对照组各33例。西药对照组予西医常规治疗,中药治疗组给予口服自拟清阳祛邪汤联合中药外敷,两组均治疗6周,观察临床疗效、甲状腺功能、红细胞沉降率(ESR)和C反应蛋白(CRP)、中医证候积分及不良反应情况。结果用药6周后中医治疗组总有效率为90.91%,明显高于西药对照组的84.85%(P<0.05);中药治疗组甲状腺体积减小程度明显优于西药对照组(P<0.05);中药治疗组能有效改善甲状腺功能,效果优于西药对照组(P<0.05);中药治疗组能有效降低ESR、CRP,效果优于西药治疗组(P<0.05);中药治疗组有效改善患者发热、颈前疼痛、颈前肿胀、心悸、乏力、急躁易怒、多汗、震颤等临床症状,效果优于西药治疗组(P<0.05)。结论自拟清阳祛邪汤联合中药外敷治疗亚急性甲状腺炎疗效确切,且不良反应轻,安全性好。

Objective:To observe the clinical efficacy of self-made Qingyang Quxie Decoction combined with topical application of TCM for subacute thyroiditis.Methods:A total of 66 eligible patients were selected and randomly divided into two groups according to random number table methods,33 cases in each group.The control group received prednisone acetate tablets as prescribed,while the treatment group received self-made Qingyang Quxie Decoction combined with topical application of TCM.Both groups were treated for 6 weeks.The clinical efficacy,thyroid function,erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP),TCM syndrome scores and adverse reactions of the two groups were observed.Results:After 6 weeks of treatment,the total effective rate of the treatment group was 90.91%,which was significantly higher than 84.85%of the control group(P<0.05);the thyroid volume reduction in the treatment group was significantly better than that of the control group(P<0.05);the treatment group effectively improved the thyroid function and the effect was better than the control group(P<0.05);the treatment group effectively reduced the ESR and CRP levels,and the effect was better than the control group(P<0.05);the treatment group effectively improved the clinical symptoms such as fever,anterior neck pain,anterior neck swelling,palpitations,fatigue,irritability,hyperhidrosis,tremor,etc.,and the effect was better than that of the control group(P<0.05).Conclusion:The self-made Qingyang Quxie Decoction combined with topical application of TCM for subacute thyroiditis has definite curative effect,mild side effects and good safety.
