清热通络方外敷联合常规疗法治疗急性痛风性关节炎30例临床观察

Clinical Observation on 30 Cases of Acute Gouty Arthritis Treated by External Application of Qingre Tongluo Fang(清热通络方)Combined with Conventional Therapy

目的:观察清热通络方外敷联合常规疗法治疗急性痛风性关节炎关节肿痛的临床疗效。方法:将64例急性痛风性关节炎患者随机分为治疗组和对照组,每组32例。对照组采用活血化瘀药静脉滴注,碳酸氢钠片、塞来昔布胶囊、醋酸泼尼松片口服治疗,治疗组在对照组基础上加用清热通络方外敷治疗。2组均以7 d为1个疗程。观察2组临床疗效,以及治疗前后关节肿胀度、关节压痛度、视觉模拟评分法(VAS)评分、中医证候积分。结果:治疗组完成30例,对照组完成32例。治疗组临床痊愈6例,显效16例,有效7例,无效1例,总有效率为96.67%;对照组临床痊愈2例,显效7例,有效19例,无效4例,总有效率为87.50%。2组比较,差异有统计学意义(P<0.05)。治疗后,2组关节肿胀度、关节压痛度、VAS评分、中医证候积分较治疗前均有改善(P<0.05);且治疗组优于对照组(P<0.05)。结论:清热通络方外敷联合常规疗法治疗急性痛风性关节炎,能有效改善局部关节肿痛症状。

Objective:To observe the clinical efficacy of Qingre Tongluo Fang(清热通络方)combined with conventional therapy in the treatment of acute gouty arthritis.Methods:Sixty-four patients with acute gouty arthritis were randomly divided into a treatment group and a control group,32 cases in each.The control group was given medicine of invigorating blood and dissolving stasis by intravenous drip,and sodium bicarbonate tablets,celecoxib capsules and prednisone acetate tablets for oral administration,and the treatment group was given Qingre Tongluo Fang based on the treatment for the control group.Both groups were treated for 7 days as a course of treatment.The clinical efficacy,joint swelling degree,joint tenderness degree,visual analogue scale(VAS)and TCM syndrome score before and after treatment were observed.Results:There were 30 cases in the treatment group and 32 cases in the control group.In the treatment group,6 cases were recovered,16 cases were markedly effective,7 cases were effective and 1 case was ineffective,with a total effective rate of 96.67%;in the control group,2 cases were recovered,7 cases were markedly effective,19 cases were effective and 4 cases were ineffective,with a total effective rate of 87.50%.The difference between the two groups was statistically significant(P<0.05).After treatment,joint swelling,joint tenderness,VAS and TCM syndrome score of the two groups were improved compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).Conclusion:Qingre Tongluo Fang combined with conventional therapy can effectively improve the symptoms of local joint swelling and pain in the treatment of acute gouty arthritis.
