CEA、CA724、CA199肿瘤标志物在胃癌诊断中的应用价值

Application value of CEA,CA724,CA199 tumor markers in the diagnosis of gastric cancer

目的探讨癌胚抗原(CEA)、糖链抗原(CA)724、CA199在胃癌诊断中的应用价值。方法将2015年1月至2016年12月我院收治的初诊、初治胃癌患者40例纳入研究组,将同期我院体检健康者40例纳入对照组。从既往检查报告中收集两组CEA、CA724、CA199表达水平并比较,分析CEA、CA724、CA199单一诊断和联合诊断胃癌的效能,同时,通过绘制ROC曲线分析CEA、CA724、CA199单一诊断和联合诊断在胃癌中的预测价值。结果研究组CEA、CA724、CA199表达水平明显高于对照组,差异具有统计学意义(P<0.05)。CEA诊断胃癌的灵敏度、阳性预测值、阴性预测值最高,特异度与CA724相同,单一诊断效能CEA>CA724>CA199。CEA+CA724诊断胃癌的特异度、阳性预测值、阴性预测值最高,灵敏度与CEA+CA724+CA199相同。CEA+CA724+CA199诊断胃癌的AUC值(0.982)最大,明显大于CEA、CA724、CA199单一诊断和两两联合诊断时的AUC值。结论胃癌患者CEA、CA724、CA199表达水平高于正常人群,CEA+CA724诊断胃癌在保证合理特异度的情况下,可提高诊断灵敏度、阳性预测值、阴性预测值,对胃癌的早期诊断、治疗具有指导意义。

Objective To investigate the applicatien value of carcinoembryonic antigen(CEA),carbohydrate antigen(CA)724 and CA199 in the diagnosis of gastric cancer.Methods From January 2015 to December 2016,40 patients with newly diagnosed and treated gastric cancer admitted in our hospital were included in the study group,and 40 healthy people with physical examination in our hospital during the same period were included in the control group.The expression levels of CEA,CA724 and CA199 were collected from the previous examination reports and compared,and the efficacy of CEA,CA724 and CA199 in single diagnosis and combined diagnosis of gastric cancer were analyzed.At the same time,the predictive value of CEA,CA724 and CA199 in single diagnosis and combined diagnosis of gastric cancer were analyzed by drawing ROC curve.Results The expression levels of CEA,CA724 and CA199 of the study group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).CEA had the highest sensitivity,positive predictive value and negative predictive value in the diagnosis of gastric cancer,and the specificity was the same as CA724,the single diagnostic efficiency was CEA>CA724>CA199.CEA+CA724 had the highest specificity,positive predictive value and negative predictive value in the diagnosis of gastric cancer,and the sensitivity was the same as CEA+CA724+CA199.The AUC value of CEA+CA724+CA199 in the diagnosis of gastric cancer was the highest(0.982),which was significantly higher than that of CEA,CA724,CA199 single diagnosis and combination diagnosis.Conclusion The expression levels of CEA,CA724 and CA199 in gastric cancer patients are higher than those in normal people.CEA+CA724 can improve the diagnostic sensitivity,positive predictive value and negative predictive value in the diagnosis of gastric cancer with reasonable specificity,which has guiding significance for the early diagnosis and treatment of gastric cancer.
