精细化管理体系用于降低难免性压疮患者压疮发生率的效果评价

Evaluation of the effect of fine management system on reducing the incidence of unavoidable pressure ulcers in patients

目的观察精细化管理体系用于降低难免性压疮患者压疮发生率的效果。方法按照接诊顺序选取北京市大兴区中西医结合医院2017年1月至2018年12月接诊的有压疮风险的300例患者作为研究对象,2017年1—12月接收的患者为对照组(n=140例),2018年1—12月接收的患者为观察组(n=160例)。对照组采用常规压疮防治模式,观察组在对照组基础上接受精细化管理。对比分析两组压疮申报率、申报合格率及压疮发生率、住院时间、住院满意率,干预前后两组压疮风险Barden评分等以评价精细化管理体系的应用效果。结果干预前,两组Barden评分比较,差异无统计学意义(P>0.05);干预后,两组Barden评分均较干预前均有显著性提高(P<0.01),观察组Barden评分显著高于对照组(P<0.01)。观察组难免性压疮申报率、申报合格率均显著高于对照组(P<0.01),压疮发生率显著低于对照组(P<0.05)。出院时观察组患者住院总满意率明显高于对照组(P<0.05),住院时间显著性短于对照组(P<0.05)。结论精细化管理体系不仅可显著降低难免性压疮患者压疮发生率,而且也有助于提高住院质量和缩短住院时间,值得推广应用。

Objective To observe the effect of fine management system in reducing the incidence of unavoidable pressure ulcers(PUs)in patients.Methods A total of 300 patients with PUs risk in Beijing Daxing District Hospital of Integrated Traditional Chinese and Western Medicine from January 2017 to December 2018 were selected according to the reception order.According to the reception number,the patients received from January to December 2017 were enrolled into the control group(n=140)and the patients received from January to December 2018 were included in the observation group(n=160).The control group received the routine prevention and treatment mode of PUs,while the observation group was treated with fine management on the basis of the control group.The declaration rate of PUs,the qualified rate of declaration,the incidence rate of PUs,the length of stay,the satisfaction rate of hospitalization,and the Barden score for PUs risk between the two groups before and after intervention were compared and analyzed to evaluate the effect of the fine management system.Results Prior to intervention,there was no significant difference in Barden score between the two groups(P>0.05),but after intervention,the Barden score in both groups was significantly higher than that before intervention(P<0.01),and the Barden score in the observation group was significantly higher than that in the control group(P<0.01).The declaration rate and the qualified rate of unavoidable PUs in the observation group were significantly higher than those in the control group(P<0.01),and the incidence of PUs in the observation group was significantly shorter than that in the control group(P<0.01).In addition,the total hospitalization satisfaction rate of the observation group was significantly higher than that of the control group(P<0.05),and the hospital stay of the observation group was significantly lower than that of the control group(P<0.01).Conclusion The fine management system can significantly reduce the incidence of PUs in patients with unavoidable PUs,and help to improve the quality of hospitalization and shorten hospital stays,which is worthy of popularization and application.
