基于授权理论的看图对话工具在糖尿病患者胰岛素注射知识宣教中的应用 被引量:2

摘要 目的研究基于授权理论的看图对话工具在糖尿病患者胰岛素注射知识宣教中的应用价值。方法选取2017年2月至2019年10月于惠东县人民医院接受治疗的186例2型糖尿病老年患者,采用随机数字表法分为两组,每组各93例。对照组行常规知识宣教,观察组采用基于授权理论的看图对话工具进行知识宣教,比较两种不同宣教方式获得效果的异同。结果干预后,观察组患者的授权能力评分和自护能力评分均高于对照组,差异有统计学意义(P<0.05);干预后两组的血糖指标均低于干预前,观察组干预后患者的空腹血糖、餐后2 h血糖、糖化血红蛋白均低于对照组,差异有统计学意义(P<0.05);经调查统计,发现观察组患者接受干预后的胰岛素注射正确率均显著高于对照组,差异有统计学意义(P<0.05);观察组患者注射胰岛素的不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论对糖尿病患者进行胰岛素注射知识宣教过程中运用基于授权理论的看图对话工具可获得理想效果,不仅能够提升患者的授权能力和自护能力,控制血糖,还可以提高患者自行注射胰岛素的操作正确率,降低注射胰岛素的不良反应风险,故值得进一步推广应用。 Objective To research the application value of the conversation map tool based on authorization theory in the propaganda and education of insulin injection knowledge for patients with diabetes.Methods A total of 186 elderly patients with type 2 diabetes mellitus admitted to Huidong People's Hospital from February 2017 to October 2019 were selected,and they were divided into the observation group(n=93)and the control group(n=93)according to the random number table method.The control group was treated with conventional propaganda and education of knowledge,while the observation group was treated with propaganda and education of knowledge by using the conversation map tool based on authorization theory.The differences and similarities of the two different propaganda and education methods were compared.Results After intervention,the scores of authorization ability and self-nursing ability of patients in the observation group were both higher than those in the control group,with statistically significant differences(P<0.05).After the intervention,the blood glucose indexes of the two groups were both lower than those before the intervention,and the fasting blood glucose,2 hours postprandial blood glucose and glycosylated hemoglobin of the patients in the observation group were all lower than those in the control group,with statistically significant differences(P<0.05).After investigation and statistics,it was found that the correct rates of insulin injection in patients of the observation group were all significantly higher than those in the control group,with statistically significant difference(P<0.05).The incidence of adverse reactions(ADRs)caused by insu lin injection in the observation group was significantly lower than that in the control group,with statistically significant difference(P<0.05).Conclusion In the process of propaganda and education of insulin injection knowledge for patients with diabetes,using the conversation map tool based on authorization theory can achieve satisfying results.Meanwhile,it can not only improve patients'authorization ability and self-nursing ability,control blood glucose,but also improve the correct rates of insulin injection by patients themselves and reduce the risk of ADRs of insulin injection.Therefore,it is worthy of further promotion and app lication.
