摘要
目的探讨数字肺技术评价中药治疗慢性阻塞性肺疾病(COPD)中气道与血管重塑的临床效果。方法选取2016年2月至2018年12月本院收治的80例COPD患者,采用随机数字表法分为对照组与研究组,各40例。研究组给予改良益气活血消癥方治疗,对照组给予玉屏风颗粒治疗。两组均进行数字肺技术扫描测定CT肺小血管截面积与肺截面积比值(%CAS)和肺功能。结果治疗后,两组%CSA<5 mm^(2)和%CSA 5~10 mm^(2)均高于治疗前,且研究组上述指标均高于对照组,差异有统计学意义(P<0.05);治疗后,两组第1秒用力呼气容积(FEV1)、FEV1与用力肺活量(FVC)的比值(FEV1/FVC)、一氧化碳的弥散量(DLCO)均高于治疗前,且研究组上述指标均高于对照组,差异有统计学意义(P<0.05)。结论采用数字肺技术评价COPD患者中肺功能、气道与血管重塑情况,证实改良益气活血消癥方治疗COPD的效果更显著。
Objective To explore the digital lung technology to evaluate the clinical effects of traditional Chinese medicine on airway and vascular remodeling in chronic obstructive pulmonary disease(COPD).Methods A total of 80 COPD patients in our hospital from February 2016 to December 2018 were selected and they were randomly divided into control group and study group,with 40 cases in each group.The study group was treated with modified Yiqi Huoxue Xiaozheng Decoction,and the control group was treated with Yupingfeng granules.Both groups underwent digital lung scanning to determine the ratio of CT pulmonary small blood vessel cross-sectional area to lung cross-sectional area(%CAS)and lung function.Results After treatment,the%CSA<5 mm^(2) and%CSA 5-10 mm^(2 )of the two groups were higher than before treatment,and the above indicators of the study group were higher than those of the control group,the difference was statistically significant(P<0.05);after treatment,the two groups were at the first second forced expiratory volume(FEV1),the ratio of FEV1 to forced vital capacity(FVC)(FEV1/FVC),and the diffusion volume of carbon monoxide(DLCO)were all higher than before treatment,and the above indicators of the study group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The digital lung technology evaluates the lung function,airway and blood vessel remodeling in COPD patients,and confirms that the modified Yiqi Huoxue Xiaozheng Decoction is more effective in treating COPD.
