探讨乙型肝炎病毒前S1抗原(Pre-SlAg)阳性检验价值及对检验准确率的影响

目的分析乙型肝炎病毒前S1抗原(Pre-SlAg)阳性的检验价值及对检验结果准确性的影响。方法选取2018年10月至2019年10月于本院治疗的乙型病毒性肝炎患者80例,在空腹状态下采集静脉血,以ELISA法测定Pre-SlAg和HBeAg,以PCR荧光定量法测定HBVDNA。观察在HBV-DNA阳性、HBeAg阳性患者中,Pre-SlAg和HBeAg、Pre-SlAg和HBV-DNA的阳性率。以HBV-DNA为金标准,计算Pre-SlAg、HBeAg的诊断准确率。结果在HBV-DNA检验结果呈阳性的患者中,Pre-SlAg阳性率为85.71%,高于HBeAg的阳性率57.14%,差异有统计学意义(P<0.05)。在HBeAg检验结果呈阳性的患者中,Pre-SlAg阳性率为80.00%,低于HBV-DNA的阳性率86.67%,但差异无统计学意义。以HBVDNA为金标准,Pre-SlAg的诊断准确率为82.86%,HBeAg的诊断准确率为64.29%。结论乙型肝炎病毒前S1抗原阳性对检验结果准确性有较高的影响,说明HBV复制活跃,临床可以此判断乙肝病毒传染性是否较强。

Objective To analyze the value of Pre-S1 Antigen Pre-SlAG positive test and the influence of Pre-S1 Antigen Pre-SlAG on the accuracy of test results.Methods 80 patients treated with viral hepatitis B in our hospital from October 2018 to October 2019 were selected,and venous blood was collected in their fasting state.Pre-SlAg and HBeAg were measured by ELISA,and quantitative PCR method.Determination of HBV-DNA by PCR fluorescence quantification method.To observe the positive rates of Pre-SlAg and HBeAg,Pre-SlAg and HBV-DNA in HBV-DNA positive and HBeAg positive patients.HBV-DNA was used as the gold standard to calculate the diagnostic accuracy of Pre-SlAg and HBeAg.Results Among patients with positive HBV-DNA test results,the Pre-SlAg positive rate was 85.71%,which was higher than the HBeAg positive rate of 57.14%,and the difference was statistically significant(P<0.05).Among the patients with positive HBeAg test results,the pre-SlAg positive rate was 80.00%,which was lower than the HBV-DNA positive rate of 86.67%,but the difference was not statistically significant.With HBV-DNA as the gold standard,the diagnostic accuracy of Pre-SlAg was 82.86%,and the diagnostic accuracy of HBeAg was 64.29%.Conclusion Positive S1 antigen before hepatitis B virus has higher impact on the accuracy of the test results, which means that HBV replication is active, clinical can use this to judgewhether the infection of hepatitis B virus is strong.
