双切口双钢板内固定对复杂胫骨平台骨折患者术后恢复的影响

Effect of double-incision double plate internal fixationon postoperative recovery of patients with complextibial plateau fracture

目的探讨双切口双钢板内固定对复杂胫骨平台骨折患者术后恢复的影响。方法选取2018年10月至2019年10月本院骨科收治的复杂胫骨平台骨折患者56例,按照随机数字表法分为两组,各28例。对照组行膝前正中直切口双钢板固定术,观察组行膝内外侧双切口双钢板内固定术,比较两组术后患肢负重时间、骨折愈合时间及术后1年Merchant评分,并用简式Fugl-Meyer运动功能评分法、功能性步行分级(FAC)评定两组下肢功能情况。结果观察组骨折愈合时间、患肢负重时间均短于对照组(P<0.05)。术后1年,观察组优良率为85.71%,显著高于对照组的71.43%,差异有统计学意义(P<0.05)。观察组FMA、FAC评分均明显高于对照组(P<0.05)。结论针对复杂胫骨平台骨折患者,实施双切口双钢板内固定,能缩短骨折愈合时间及患肢负重时间,效果显著。

Objective To investigate the effect of double-incision double-plate internal fixation on postoperative recovery of patients with complex tibial plateau fractures.Methods From October 2018 to October 2019,56 patients with complicated tibial plateau fractures admitted to the orthopedics department of our hospital were randomly divided into two groups,with 28 cases in each group.The control group was treated with double plate fixation,the observation group was treated with double incision and double plate internal fixation.The weight-bearing time,fracture healing time and merchant score were compared between the two groups.The lower limb function of the two groups was evaluated by simplified Fugl-Meyer motor function score and functional walking classification(FAC).Results The fracture healing time and the weight of the limbs in the observation group were shorter than those in the control group(P<0.05).The excellent and good rate in the observation group was 85.71%,which was significantly higher than 71.43%in the control group(P<0.05).The FMA and FAC scores of the observation group were significantly higher than those of the control group(P<0.05).Conclusion For patients with complex tibial plateau fractures,double-incision double-plate internal fixation can shorten the time of fracture healing and the weight-bearing time of the affected limb.
