磁共振联合3D-TOF-MRA对急性基底动脉尖综合征的诊断价值

Diagnostic value of magnetic resonance imagingcombined with 3D-TOF-MRA in acute basilar arteryapex syndrome

目的探讨磁共振联合三维时间飞跃法磁共振血管成像(3D-TOF-MRA)对急性基底动脉尖综合征(TOBS)的诊断价值。方法选取2018年1月至2019年6月本院接收的68例疑似为急性TOBS患者,所有患者均给予磁共振及3D-TOF-MRA检查,以数字减影脑血管造影(DSA)检查结果为金标准,分析磁共振联合3D-TOF-MRA对急性基底动脉尖综合征的诊断价值。结果 68例疑似急性TOBS患者中,经DSA检查确诊患者有48例,占比70.59%。以DSA检查结果为金标准,联合检查的疾病检出率(97.92%)高于3D-TOF-MRA(85.42%)、磁共振检查(75.00%),差异均有统计学意义(P<0.05);联合检查的灵敏度、准确度、阴性预测值均高于3D-TOF-MRA、磁共振检查,差异有统计学意义(P<0.05);且联合检查特异度、阳性预测值略高于3D-TOF-MRA、磁共振检查,但差异无统计学意义。结论磁共振联合3D-TOF-MRA对急性TOBS的诊断价值较高,可提高疾病检出率及准确度,具有较高的灵敏度、特异度,为临床早期治疗提供重要参考。

Objective To evaluate the diagnostic value of magnetic resonance imaging(MRI)combined with three-dimensional time jump magnetic resonance angiography(3D-TOF-MRA)in acute basilar artery syndrome(TOBS).Methods From January 2018 to June 2019,68 patients with suspected acute TOBS were selected.All patients were examined by MRI and 3D-TOF-MRA.The diagnostic value of MRI combined with 3D-TOFMRAin acute basilar artery cusp syndrome was analyzed according to the gold standard of digital subtraction cerebral angiography(DSA).Results 68 cases of suspected acute TOBS in the patients,48 cases were diagnosed by DSA and 70.59%.The results of DSA were the gold standard,the rate of the combined examination(97.92%)was higher than that of the 3D-TOF-MRA(85.42%)and the magnetic resonance(75.00%),the difference was statistically significant(P<0.05).The sensitivity,accuracy and negative predictive value of the combined examination were higher than that of the 3DTOF-MRA and the magnetic resonance examination,the difference was statistically significant(P<0.05).The specific degree of the combined examination was slightly higher than that of the 3D-TOF-MRA and the magnetic resonance test,the difference was not significant.Conclusion MRI combined with 3D-TOF-MRA has high diagnostic value for acute TOBS,can improve the detection rate and accuracy of the disease,has higher sensitivity and specificity,and provides an important reference for clinical early treatment.
