超短波联合骨盆牵引治疗对腰椎间盘突出症患者疼痛症状及QOL评分的影响

Effect of Ultrashort Wave Physiotherapy Combined with Pelvic Traction on Pain Symptoms and Quality of Life Score in Patients with Lumbar Disc Herniation

目的探讨超短波联合骨盆牵引治疗对腰椎间盘突出症患者疼痛症状及生活质量(QOL)评分的影响。方法回顾性分析2018年5月至2019年5月承德医学院附属医院骨伤科收治的84例腰椎间盘突出症患者的临床资料,依据治疗方法不同分为对照组和研究组,每组42例。对照组采用骨盆牵引,研究组采用骨盆牵引联合超短波治疗。比较两组患者的临床疗效,治疗前后疼痛症状量化评分、QOL评分以及不良反应发生情况。结果研究组治疗总有效率高于对照组[95.24%(40/42)比78.57%(33/42)](P<0.05)。治疗前后腰痛、压痛、下肢麻木或疼痛、放射痛、诱发痛的疼痛症状量化评分及QOL评分的主效应差异有统计学意义(P<0.05);不考虑测量时间,两组间各项疼痛症状量化评分、QOL评分的主效应差异有统计学意义(P<0.05);各项疼痛症状量化评分、QOL评分的组间与时点间存在交互作用(P<0.05),两组治疗前后各项疼痛症状量化评分、QOL评分的变化幅度不同,研究组变化更明显。治疗后,对照组和研究组不良反应发生率分别为0和2.4%(1/42),两组比较差异无统计学意义(P>0.05)。结论超短波联合骨盆牵引治疗可显著提高腰椎间盘突出症患者的临床治疗效,提高QOL评分,减轻患者的疼痛症状,且安全性高。

Objective To discuss the effect of ultrashort wave therapy combined with pelvic traction on pain symptoms and quality of life(QOL)score of patients with lumbar disc herniation.Methods The clinical data of 84 patients with lumbar disc herniation admitted to the Department of Orthopedics and Traumatology,Affiliated Hospital of Chengde Medical University from May 2018 to May 2019 were retrospectively analyzed.They were divided into a control group and a study group according to different treatment methods,42 cases each.The control group was treated with pelvic traction,and the study group was treated with pelvic traction combined with ultrashort wave.The clinical efficacy and adverse reactions,and the pain symptom quantification,QOL score before and after treatment of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group[95.24%(40/42)vs 78.57%(33/42)](P<0.05).The differences of main effects of quantitative scores and QOL scores of low back pain,tenderness,lower extremity numbness or pain,radiating pain,induced pain before and after treatment were statistically significant(P<0.05);regardless of measurement time,the main effect differences in the pain symptoms quantitative score and QOL score between the two groups were statistically significant(P<0.05);there interactions in the pain symptoms quantitative scores and QOL scores between groups and time points(P<0.05);the changing amplitudes in the pain symptoms quantitative score and QOL score were different,and the changes in the study group were more obvious.After treatment,the adverse reaction rates of the control group and the study group were 0 and 2.4%(1/42)resepctively,and there was no statistically significant difference between the two groups(P>0.05).Conclusion Ultrashort wave combined with pelvic traction treatment significantly improves the clinical effect for patients with lumbar disc herniation,which can improve the QOL scoreand reduce the pain symptoms of the patients,with high safety.
