摘要
目的探讨临床药师干预对呼吸内科住院患者抗菌药物使用情况的影响。方法选取长海医院虹口院区呼吸内科2013年7月至2014年6月住院患者为对照组,2015年7月至2016年6月住院患者为干预组,干预方法是临床药师参与呼吸内科抗菌药物用药过程,提取患者抗菌药物使用资料并进行统计分析。结果与对照组比较,干预组患者二联和三联及以上抗菌药物使用率、住院费用、用药频度(DDDs)、药物利用指数(DUI)、抗菌药物使用强度(AUD)明显下降;单联使用抗生素、药敏试验送检率明显提高。干预组无适应证抗菌药物的应用、不合理品种选择、用法用量错误、不合理的联合用药、错误的换药和疗程的发生率较对照组均明显降低。结论药师干预对呼吸内科住院患者合理应用抗菌药具有显著作用。
Objective To investigate the impact of clinical pharmacist intervention on rational use of antibiotics in the inpatients of respiratory medicine.Methods The inpatients admitted into the Respiratory Department of the Hongkou Branch,Changhai Hospital from July 1,2013 to June 31,2014 were designated as the control group,and those hospitalized in the same department from July 1,2015 to June 31,2016 were designated as the intervention group.Clinical pharmacists were directly involved in the patient medication process,and clinical medication data concerning the use of antibiotics were collected for statistical analysis.Results As compared with those of the control group,the application rate,hospitalization cost,frequencies of medication(DDD),drug utilization index(DUI)and AUD in the application of bigeminal,triple and tetra or more antibiotic combinations in the patients of the intervention group dropped sharply,while the application rate of one single antibiotic and the rate of drug allergy tests increased significantly.Antibiotic use without indication,improper choice of drug categories,irrational dosage,improper combined medication,wrong switch of drugs and treatment courses reduced significantly in the intervention group,as compared with those of the control group.Conclusion Pharmacist intervention could play a significant role in rational application of antibiotics in the inpatients of the respiratory department.
