基于疾病相关分组的层级式中医师临床诊治能力评价研究

Research on hierarchical evaluation of clinical ability of traditional Chinese medicine physicians based on DRGs

目的:在疾病相关分组理论框架下引入中医药特色指标开展层级式中医师临床诊治能力评价。方法:选取中医医院S科室全体中医师作为研究对象,通过电子病历系统获取2018年指标数据,采用综合指数法对不同层级的中医师临床诊治能力进行量化评分。结果:各层级中医临床诊治能力最高排序分别为正高级中医师C、副高级中医师E、中级中医师G和初级中医师K;不同层级间,副高级中医师D和E达到了正高级中医师临床诊治水平;综合评价显示,该专科病种难度与中医药参与程度呈反比,应加强中医药运用能力。结论:评价指标体系适用性强,能够体现不同成长阶段应具备的临床诊治能力,有利于引导中医师提升临床诊治水平及中医药服务能力。

Objective:To introduce characteristic indexes of traditional Chinese medicine under the framework of diagnosis related groups theory to carry out hierarchical evaluation of clinical ability of traditional Chinese medicine physicians.Methods:All Chinese medicine physicians in the S department of the Chinese medicine hospital were selected as the research object.The 2018 index data was obtained through the electronic medical record system,and the comprehensive index method was used to quantitatively score the clinical ability of Chinese medicine physicians at different levels.Results:The highest ranks of clinical ability of Chinese medicine at all levels were the senior Chinese physician C,the deputy senior Chinese physician E,the intermediate Chinese physician G and the junior Chinese physician K;among the different levels,the deputy senior Chinese physicians D and E reached the senior Chinese physician the level of clinical ability;comprehensive evaluation shows that the difficulty of this specialty disease is inversely proportional to the degree of participation of traditional Chinese medicine,and the ability to apply traditional Chinese medicine should be strengthened.Conclusion:The appraisal index system has strong applicability,can reflect the clinical capabilities that should be possessed in different growth stages,and is conducive to guiding Chinese medicine physicians to improve the level of clinical ability and traditional Chinese medicine service capabilities.
