持续被动运动在桡骨远端骨折内固定术后康复中的效果评价

Evaluation of the effect of continuous passive exercise on postoperative rehabilitation of distal radius fractures

目的分析评价持续被动运动在桡骨远端骨折内固定术后康复中的运用效果。方法选择2014年6月-2015年12月收治的桡骨远端不稳定骨折128例作为研究对象,入组患者均接受内固定术治疗,根据随机数表法将患者分为观察组(n=63)和对照组(n=65),对照组术后接受主动活动的康复训练治疗,观察组术后应用CPM机进行持续被动康复性训练,比较两组患者康复训练效果及训练前后疼痛感的改善情况。结果观察组与对照组康复训练后7 d、14 d的Gartland-Werley评分比较分别为(6.54±1.20 vs 14.66±3.02)、(2.01±0.16 vs 7.99±2.84);伤肢肿胀情况比较分别为(11.24±1.35 vs 21.05±3.44)、(6.05±0.74 vs 17.35±2.66);VAS评分比较分别为(2.22±0.30 vs 3.87±0.38)、(1.25±0.11 vs 3.02±0.32)(均P<0.05)。术后随访1个月,观察组并发症发生率3.17%,明显低于对照组16.92%(P<0.05)。结论持续被动运动对桡骨远端骨折内固定术后康复有良好的促进作用,消肿、缓解疼痛效果佳,能有效改善患者腕关节功能,安全性高,值得临床推广应用。

Objective To evaluate the effect of continuous passive exercise on postoperative rehabilitation of distal radius fractures.Methods From June 2014 to December 2015,128 patients with unstable distal radial fracture who were treated in Orthopedic Clinic of our hospital were enrolled in this study.All patients were treated with internal fixation.According to the random number table,(N=63)and control group(n=65).Patients in the control group received active rehabilitation exercise after operation.CPM machine was used in the observation group for continuous passive rehabilitation training.The recovery of the patients in the two groups was compared in training effect and improvement of pain before and after the training.Results The scores of Gartland-Werley scores on the 7th and 14th days after the rehabilitation training in the observation group and the control group were(6.54±1.20 vs 14.66±3.02),(2.01±0.16 vs 7.99±2.84),and the swelling of the injured limb were(11.24±1.35 vs 21.05±3.44),(6.05±0.74 vs 17.35±2.66).The VAS scores were(2.22±0.30 vs 3.87±0.38)and(1.25±0.11 vs 3.02±0.32)(all P<0.05).After 1 month of follow-up,the incidence of complications in the observation group was 3.17%,which was significantly lower than that in the control group,16.92%(P<0.05).Conclusion Persistent passive exercise can promote the rehabilitation of the distal radius fracture.It has good swelling and pain relief,can effectively improve the wrist function of patients,has high safety,and is worthy of clinical promotion.
