摘要
目的探究在截骨延长成骨过程中联合应用BMP-2和BMP-7对成骨的促进作用。方法采用健康成年日本大耳白兔30只,完全随机分为三组。A组作为对照组正常牵拉不做其他处理;B组在截骨后于截骨处加入rhBMP-2药片;C组在截骨后于截骨处加入rhBMP-2药片,7 d后在延长区经皮注射rhBMP-7溶解液。在术后第7天、第38天分别拍摄三组模型的右侧胫骨全长X线片;术后38 d取三组标本分别测量三组延长区及远近截骨端的骨密度;将取下的三组标本进行HE染色,光镜下观察。结果术后第38天X线观察:A组见截骨两端形成一定量的骨组织,延长区的骨痂影密度低,有透亮区;B组延长区新生骨组织较为均匀,可见骨折线;C组骨延长区形成的骨组织影密度高于其他两组,未见明显骨折线。使用SPSS 17.0软件对各组的骨密度进行统计学分析:B、C组的骨密度测定值明显高于A组(P<0.05),C组骨密度测定值优于B组(P<0.05);术后第38天HE染色:A组骨小梁内骨细胞稀少,B、C组骨小梁密度体积增大明显,骨痂向骨性骨痂改建明显,髓腔再通。结论BMP-2能促进兔胫骨牵张成骨的形成和愈合;BMP-7可加快启动软骨内成骨,使软骨内成骨时间提前,联合应用BMP-2和BMP-7比单纯应用BMP-2可更快速地形成新的骨组织,二者在成骨上具有协同作用。
Objective To investigate the effect of BMP-2 and BMP-7 on osteogenesis in osteotomy and lengthening osteogenesis.Methods Totally 30 healthy adult Japanese white rabbits were randomly divided into 3 groups.Group A had normal traction without any other treatment,as control group;group B was added rhBMP-2 tablets after osteotomy;group C after osteotomy was added rhBMP-2 tablets after osteotomy,7 days later,in the extended area the rhBMP-7 solution was injected into the area.On the seventh day and thirty-eighth day after the operation,the full length X-ray films of the right tibia were taken respectively in the three groups,and three groups were taken to measure the bone density in the three groups and the distal end of the osteotomy at 38th day after the operation,and three groups of specimens were made with HE staining and observed under light microscope.Results Thirty-eighth days after operation,X ray observation:in group A,a certain amount of bone tissue was formed at both ends of the osteotomy.In group B,the new bone tissue in the extended area was more uniform and the fracture line was visible.The bone tissue shadow density in C group was higher than that of the other two groups,and no obvious fracture line was found.The bone mineral density in group B and C was significantly higher than that of group A(P<0.05),and the bone mineral density in group C was better than that of group B(P<0.05).The bone mineral density in group C was better than that of group B(P<0.05),and HE staining at thirty-eighth days after operation was found in the A group bone trabecular bone fine.In the B and C groups,the density and volume of trabecular bone increased significantly.The callus was reconstructed to the callus and the pulp cavity was recanalization.Conclusion BMP-2 can promote the formation and healing of distraction osteogenesis;BMP-7 can advance the initial time of cartilage osteogenesis.The combined application of BMP-2 and BMP-7 can form a new callus more rapidly than the simple application of BMP-2.Those two has a synergistic effect in promoting osteogenesis.
