基于文本挖掘的中医治疗甲状腺功能正常桥本甲状腺炎临床研究用药特征分析

Analysis of the Characteristics of Drugs Used in Clinical Studies of TCM Treatment of Normal Thyroid Function Hashimoto′s Thyroiditis Based on Text Mining

目的探索中医药治疗甲状腺功能正常桥本氏甲状腺炎的用药特征。方法检索中国知网、万方数据库2010年—2019年相关临床研究文献,从频数、构成比、药性、药味、归经和功效角度分析用药特征,采用主成分和聚类分析寻找常用中药组合。结果纳入有效文献21篇,药物56味。频次≥2次有34味中药,累积构成比88.2%;频次≥5次有12味,最常用中药分别为夏枯草、黄芪和白术(频次皆>10次)。寒性药和温性药、甘味药和苦味药居多,归经偏于脾、肝和肺。功效方面,频次最多的是补虚药,其次为清热药。常用中药组合分别为夏枯草-黄芪-香附-生地黄、白芍-当归-柴胡、白花蛇舌草、玄参-贝母和甘草-白术-茯苓。结论甲状腺功能正常桥本甲状腺炎用药呈现寒温并用、攻补兼施,甘药补中以扶正、苦药泄热以祛邪的特征。

Objective To explore the characteristics of traditional Chinese medicine in the treatment of Hashimoto′s thyroiditis with normal thyroid function.Methods Retrieved relevant clinical observation literature from CNKI and Wanfang from 2010 to 2019,analyzed the characteristics of medications from the perspectives of frequency,composition ratio,medicinal properties,medicinal taste,meridian and efficacy,and used principal component and cluster analysis to find common Chinese medicine combinations.Results 21 valid documents and 56 medicines were sorted out.There are 34 traditional Chinese medicines with frequency≥2,and the cumulative composition ratio is 88.2%;with frequency≥5,there are 12 traditional Chinese medicines.The most commonly used traditional Chinese medicines are Prunella vulgaris,Astragalus,and Atractylodes macrocephala(with frequency>10).Cold and warm medicines,sweet medicines and bitter medicines are mostly,and the meridians are biased towards the spleen,liver and lung.In terms of efficacy,tonic drugs are the most frequently used,followed by antipyretic drugs.Five commonly used combinations of traditional Chinese medicines are found,namely Prunella vulgaris-Astragalus-Cermanthus-Rhizoma Rehmanniae,Radix Paeoniae Rubra-Angelica-Bupleurum,Hedyotis diffusa,Scrophulariaceae-Fritillaria and Licorice-Atractylodes-Poria.Conclusion Hashimoto′s thyroiditis medication with normal thyroid function is characterized by the combination of cold and warm,attacking and tonic,sweet medicine to nourish the body,and bitter medicine to relieve heat to reach evil.
