血IL-16、CA199在子宫内膜异位症的诊断、病情程度中的价值研究

Study on the value of IL-16 and CA199 in the diagnosis and severity of endometriosis

目的:检测子宫内膜异位症(Endometriosis,EMT)患者血IL-16、CA199水平,评估其在诊断、病情程度中的价值研究。方法:选择我院2019年6月至2020年3月收治并行腹腔镜治疗的75例EMT患者为EMT组,根据美国生育协会分期标准分为轻度组(I~II期,40例)和重度组(III~IV期,35例);另选择同期行健康体检的75例为对照组,分别采用酶联免疫法和电化学发光免疫检测腹腔镜手术当日空腹静脉血IL-16和CA199水平,比较不同患者的表达差异;同时分析单独及联合检测在EMT中的诊断价值。结果:EMT组患者的血IL-16、CA199水平均明显高于对照组(P<0.05);EMT轻度组患者血IL-16、CA199水平均明显低于EMT重度组(P<0.05);血IL-16联合CA199检测诊断EMT的灵敏度为87.5%,特异度为79.3%,曲线下面积(Area under the curve,AUC)为0.898(95%CI:0.805~0.956)。结论:血IL-16、CA199联合检测能够提高EMT的诊断效能,且表达水平随病情严重程度增高。

Objective:To detect the blood levels of IL-16 and CA199 in patients with endometriosis(EMT),and to evaluate its value in diagnosis and disease severity.Methods:Seventy-five EMT patients who were admitted to our hospital from June 2019 to March 2020 and were treated with laparoscopy were selected as the EMT group.According to the American Fertility Association staging standard,they were divided into mild group(I-II stage,40 cases)and severe group(phase III-IV,35cases).In addition,75 patients who underwent physical examination during the same period were selected as the control group.The expression levels of IL-16 and CA199 of fasting venous blood on the day of laparoscopic surgery were measured by enzyme-linked immunoassay and electrochemiluminescence immunoassay,respectively.Also the differences in expression levels were compared.Simultaneously,the diagnostic value of single and combined detection in EMT was analyzed.Results:The blood levels of IL-16 and CA199 in the EMT group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The blood IL-16 and CA199 levels of patients in the mild EMT group were significantly lower than those in the severe EMT group,and the difference was statistically significant(P<0.05).The sensitivity of blood IL-16 combined with CA199 to detect EMT was 87.5%,the specificity was 79.3%,and the area under the curve(Area under the curve,AUC)was 0.898(95%CI:0.805~0.956).Conclusion:The combined detection of IL-16 and CA199 in blood can improve the diagnostic efficiency of EMT,and the expression level increases with the severity of the disease.
