血清瘦素与新诊断2型糖尿病关系的临床研究

Clinical study on the relationship between serum leptin and newly diagnosed type 2 diabetes

目的对新诊断2型糖尿病患者与非糖尿病患者的血清瘦素水平与各代谢指标进行比较分析,探讨血清瘦素在新诊断2型糖尿病发生发展中的作用。方法选取2018年12月—2019年12月在我院就诊的新诊断的2型糖尿病患者169例为观察组,选择在体检中心进行健康体检的空腹血糖<6.1 mmol/L,糖化血红蛋白<6.5%的35~65岁成年人143例为正常对照组。分析血清瘦素与新诊断2型糖尿病的关系。结果胰岛素抵抗指数(HOMA-IR)、体质量指数(BMI)是血清瘦素的显著影响因素;血清瘦素(LP)、体质量指数(BMI)、糖化血红蛋白(HbA1c)、甘油三脂(TG)为胰岛素抵抗指数的显著影响因素。结论新诊断2型糖尿病患者具有高瘦素水平、胰岛素抵抗、脂代谢紊乱等特点。高瘦素水平与胰岛素抵抗在2型糖尿病发生过程中相互影响,并在2型糖尿病并发症的发展过程中起重要作用。

Objective To explore the role of serum leptin(LP)in the development of newly diagnosed type 2 diabe⁃tes by analyzing and comparing serum leptin level and various metabolic indicators between type 2 diabetes patients and non type 2 diabetes people.Methods Patients with newly diagnosed type 2 diabetes who were admitted to our hospital from December 2018 to December 2019 were selected as the observation group.143 cases in the control group included adults aged 35~65 with fasting blood glucose(FBG)<6.1mmol/L and glycated hemoglobin A1c(HbA1c)<6.5%.The rela⁃tionship between LP and newly diagnosed type 2 diabetes was analyzed.Results Insulin resistance index(HOMA-IR)and body mass index(BMI)were the main influencing factors of LP.LP,BMI,HbA1c and triglyceride(TG)were the main influencing factors of HOMA-IR.Conclusion Newly diagnosed type 2 diabetes patients have the characters of high LP level,insulin resistance and lipid metabolism disorders.High LP level interact with insulin resistance in type 2 diabetes and play an important role in the development of type 2 diabetes.
