俯卧位联合抚触按摩对新生儿黄疸蓝光照射时哭闹及生长发育的影响

Effect of prone position combined with touching massage on crying and development at the time of neonatal jaundice under blue light irradiation

目的探讨俯卧位联合抚触按摩对新生儿黄疸(NJ)蓝光照射时哭闹及生长发育的影响。方法选择2019年2月—8月期间于我院行蓝光照射治疗及常规护理的40例NJ患儿纳入对照组,选择2019年9月—2020年3月期间于我院行蓝光照射治疗及俯卧位联合抚触按摩的41例NJ患儿纳入观察组。比较两组患儿胆红素水平[经皮测胆红素(TcB)、血清总胆红素(TB)]、黄疸症状改善时间、蓝光照射时哭闹情况及生长发育情况。结果干预后,两组TcB、TB较干预前降低,观察组低于对照组(P<0.05);观察组首次排便、黄疸消退及胎便转黄时间短于对照组(P<0.05);观察组患儿睡眠时间、体重增加程度高于对照组,哭闹次数低于对照组(P<0.05)。结论NJ患儿蓝光照射时采取俯卧位联合抚触按摩,可显著降低患儿胆红素水平,增加蓝光照射时患儿睡眠时间,减少哭闹次数,促进患儿生长发育。

Objective To explore the effect of prone position combined touching massage on crying and develop⁃ment at the time of neonatal jaundice(NJ)under blue light irradiation.Methods 40 cases of NJ baby who underwent blue light irradiation therapy and routine care in our hospital from February to August 2019 were selected into the control group.41 cases of NJ baby who underwent blue light irradiation treatment and touching massage in our hospital from Sep⁃tember 2019 to March 2020 were selected into the observation group.The bilirubin levels[transcutaneous bilirubin(TcB),serum total bilirubin(TB)],time of jaundice symptoms improvement,crying and development during blue light irradiation were compared between the two groups.Results After intervention,the TcB and TB of the two groups were lower than those before intervention,and those in the observation group were lower than those of the control group(P<0.05).The first defecation,jaundice disappeared and meconium turning yellow time in the observation group were shorter than those of the control group(P<0.05).Children’s sleep time and weight gain in the obsevation goup were higher than those of the control group,but the number of crying was lower than that of the control group(P<0.05).Conclusion The prone position com⁃bined with touching massage at the time of neonatal jaundice under blue light irradiation can significantly reduce the level of bilirubin,increase the sleep time,reduce the frequency of crying and promote the growth and development of baby.
