摘要
目的探讨进展期脑梗死患者血清白细胞介素17及含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4与颈动脉粥样硬化斑块性质的相关性。方法将122例进展性脑梗死患者依据彩色多普勒超声检测结果分为无斑块组(n=30)、稳定斑块组(n=51)和不稳定斑块组(n=41)。检测3组患者血清白细胞介素17和含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4水平,分析其与进展性脑梗死患者颈动脉粥样硬化斑块性质的相关性。结果入组患者检出颈动脉粥样硬化斑块92例,检出率为75.4%。不稳定斑块组及稳定斑块组血清白细胞介素17及含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4水平均显著高于无斑块组(P<0.01),不稳定斑块组显著高于稳定斑块组(P<0.01)。白细胞介素17及含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4水平为不稳定型斑块形成的独立危险因素(P<0.01)。血清白细胞介素17、含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4预测进展性脑梗死患者斑块稳定性的受试者操作特性曲线下面积分别为0.847和0.947。结论颈动脉粥样硬化斑块进展期脑梗死患者血清白细胞介素17和含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶4水平明显升高,且与颈动脉粥样硬化斑块的性质有一定的相关性,有可能作为预测进展性脑梗死患者颈动脉粥样硬化斑块性质的血清指标。
Objective To investigate the correlation between serum interleukin 17(IL-17),4 ADAM metallopeptidase with thrombospondin type 1 motif(ADAMTS4)levels and the properties of carotid atherosclerotic plaque in patients with advanced cerebral infarction.Methods A total of 122 patients with advanced cerebral infarction were divided into plaque-free group(n=30),stable plaque group(n=51)and unstable plaque group(n=41)based on the results of color Doppler ultrasound.Serum levels of IL-17 and ADAMTS4 were detected in the three groups of patients,and their correlation with the properties of carotid atherosclerotic plaques in patients with advanced cerebral infarction was analyzed.Results A total of 92 cases of carotid atherosclerotic plaque were detected in the enrolled patients,and the detection rate was 75.4%.The levels of serum IL-17 and ADAMTS4 in the unstable plaque group and the stable plaque group were significantly higher than those in the no plaque group(P<0.01),and the unstable plaque group was significantly higher than the stable plaque group(P<0.01).IL-17 and ADAMTS4 were independent risk factors for unstable plaque formation(P<0.01).The area under the ROC curve of IL-17 and ADAMTS4 predicting plaque stability in patients with advanced cerebral infarction were 0.847 and 0.947,respectively.Conclusions The expression levels of serum IL-17 and ADAMTS4 in patients with advanced cerebral infarction of carotid atherosclerotic plaques are significantly increased,and are related to the properties of carotid atherosclerotic plaques,it may be used as a serum index to predict the properties of carotid atherosclerotic plaque in patients with advanced cerebral infarction.
