摘要
目的探讨左卡尼汀联合强精汤治疗男性不育的临床疗效。方法将96例男性不育患者按照随机数字表法分为研究组与对照组,各48例。两组均给予左卡尼汀口服治疗,研究组在此基础上联合强精汤治疗,观察3个月。比较两组临床疗效,治疗前后两组精液质量(液化时间、精子密度、精子总数、活率、直线运动比率)、精浆微量元素(锌、铜、铁)水平、促卵泡生成素、垂体泌乳素、睾酮、骨钙素水平,以及两组用药期间不良反应发生率。结果治疗后两组临床疗效比较差异无统计学意义(P>0.05);两组精液液化时间均较治疗前显著缩短(P<0.05或0.01),精子密度、精子总数、活率、直线运动比率均较治疗前显著升高(P<0.01),研究组较对照组变化更显著(P<0.05或0.01)。治疗后两组精浆锌、铜水平均较治疗前显著升高(P<0.01),铁水平均较治疗前显著降低(P<0.01),研究组较对照组变化更显著(P<0.05或0.01)。治疗后两组促卵泡生成素、垂体泌乳素水平均较治疗前显著降低(P<0.01),睾酮、骨钙素水平均较治疗前显著上升(P<0.01),研究组较对照组变化更显著(P<0.05或0.01)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论左卡尼汀联合强精汤治疗男性不育效果显著,能够有效提升精液质量,改善精浆微量元素及血清生殖激素,安全性较高。
Objective To study the curative effect of levocarnitine combined with Qiangjing decoction in the treatment of male infertility.Methods A total of 96 male infertile patients were divided into study group and control group according to the random number table method,with 48 cases in each group.Both groups were given oral levocarnitine treatment,and the study group was combined with Qiangjing decoction on this basis.Compare the clinical efficacy of the two groups,compare the semen quality(liquefaction time,sperm density,total number of sperm,viability,linear motion ratio),levels of seminal plasma trace elements(zinc,copper,iron),follicle-stimulating hormone(FSH),prolactin(PRL),testosterone(T),osteocalcin(OC)of the two groups before and after treatment.The incidence of adverse reactions during the treatment of the two groups was compared.Results There was no significant difference in clinical efficacy between the two groups(P>0.05).After treatment,the semen liquefaction time of the two groups was significantly shorter than that before treatment(P<0.05 or 0.01).The sperm density,total number of sperm,survival rate,and linear motion ratio were all significantly higher than before treatment(P<0.01).The study group changed more significantly than the control group(P<0.05 or 0.01).After treatment,the levels of zinc and copper in seminal plasma of the two groups were significantly higher than those before treatment(P<0.01),and the levels of iron were significantly lower than before treatment(P<0.01).The changes in the study group were more significant than those in the control group(P<0.05 or 0.01).After treatment,the levels of FSH and PRL in the two groups were significantly lower than before treatment(P<0.01),and the levels of T and OC were significantly higher than before treatment(P<0.01).The study group had more changes than the control group(P<0.05 or 0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Levocarnitine combined with Qiangjing decoction is effective in treating male infertility.It can effectively improve semen quality,improve seminal plasma trace elements and serum reproductive hormones,and has high safety.
