孕妇血清甲胎蛋白检验在产前监测畸形和低智胎儿中的价值

Value of Maternal SerumαFetoprotein Test in Prenatal Monitoring of Abnormal and Hypointelligent Fetuses

目的探讨孕妇血清甲胎蛋白检验在产前监测畸形和低智胎儿中的临床价值。方法选取2017年1月至2019年1月在本院行产检且检出血清甲胎蛋白阳性的70例孕妇作为阳性组,另选择同期在本院产检的70例血清甲胎蛋白阴性孕妇作为阴性组。根据产前影像学诊断或分娩后随访结果,统计两组是否出现畸形和低智胎儿,并对比畸形、低智胎儿与正常胎儿的血清甲胎蛋白水平差异。结果阴性组孕妇产前影像学检查或分娩后均未见畸形和低智胎儿。阳性组孕妇产前影像学检查发现,22例神经系统异常(31.43%),8例其他畸形者(11.42%);追踪妊娠结局及随访显示,确诊低智胎儿20例(28.57%),畸形8例(11.42%)。结论孕妇血清甲胎蛋白检验阳性者存在较高的畸形和低智胎儿风险,在产前监测可提升早期筛查效果,为该类异常产前诊断提供重要价值。

Objective To explore the clinical value of maternal serum alpha fetoprotein test in prenatal monitoring of abnormal and hypointelligent fetuses.Methods Seventy pregnant women who were tested for the serum AFP in our hospital from January 2017 to January 2019 were selected as the positive group,and 70 cases of serum AFP negative pregnant women were selected as the negative group.According to the prenatal imaging diagnosis or followup after delivery,the difference of serum alpha fetoprotein between the two groups was compared.Result In the negative group,there was no abnormal or hypointelligent fetus in prenatal imaging examination or after delivery.In the positive group,22 cases of nervous system abnormalities(31.43%)and 8 cases of other malformations(11.42%)were found by prenatal imaging examination;follow up of pregnancy outcomes and follow-up showed that 20 cases(28.57%)were diagnosed as hypointelligence fetus and 8 cases(11.42%)were diagnosed as malformation.Conclusion Pregnant women with positive serum alpha fetoprotein test have higher risk of malformation and hypointelligence fetus.Prenatal monitoring can improve the early screening effect and provide important value for prenatal diagnosis of such abnormalities.
