血浆纤维蛋白原和D-二聚体对结核性渗出性胸膜炎的意义

Significance of Plasma Fibrinogen and D-dimer in Tuberculous Exudative Pleurisy

目的探讨血浆纤维蛋白原和D-二聚体对结核性渗出性胸膜炎的意义。方法选取2018年1月至2019年1月在沈阳市第十人民医院就诊的134例进行胸腔镜诊断的胸腔积液患者,取空腹静脉血,检测血浆FIB、D-二聚体表达水平,按照病理检查结果将其分为结核性渗出性胸膜炎组(86例)和恶性胸腔积液组(48例),比较两组患者的血浆FIB、D-二聚体水平以及FIB阳性率、血浆D-二聚体阳性率。结果结核性渗出性胸膜炎组血浆FIB、D-二聚体表达水平均高于恶性胸腔积液组(P<0.05);结核性渗出性胸膜炎组FIB阳性率(77.91%)、血浆D-二聚体阳性率(90.70%)高于恶性胸腔积液组的43.75%、60.42%(P<0.05)。结论在临床上可通过血浆FIB、D-二聚体表达水平来区别结核性渗出性膜炎与恶性胸腔积液。

Objective To investigate the significance of plasma fibrinogen and D-dimer in patients with tuberculous exudative pleurisy.Methods Between January 2018 and January 2019 in Shenyang Tenth People’s Hospital of thoracoscopy in the diagnosis of 134 cases of patients with pleural effusion,fasting venous blood and detect the plasma FIB,D-dimer expression level,according to the pathological examination results will be divided into the tuberculous exudative pleurisy group(86 cases)and malignant pleural effusion group(48 cases)and comparison of two groups of patients with plasma FIB,D-dimer level and FIB positive rate,plasma D-dimer positive rate.Results The levels of plasma FIB and D-dimer in patients with tuberculous exudative pleurisy were higher than those in patients with malignant pleural effusion(P<0.05).The positive rate of FIB and D-dimer in patients with tuberculous exudative pleurisy(77.91%)and plasma(90.70%)were higher than those in patients with malignant pleural effusion(43.75%and 60.42%,P<0.05).Conclusion The expression levels of FIB and D-dimer in plasma can be used to distinguish tuberculous exudative membranitis from malignant pleural effusion.
