多参数磁共振对不同分型乳腺癌的诊断分析

Diagnosis and analysis of multi-parameter magnetic resonance for different types of breast cancer

目的分析多参数磁共振成像(MRI)技术在不同分型乳腺癌中的诊断价值。方法选取2016年12月至2018年12月新疆维吾尔自治区中医医院(新疆医科大学附属中医医院)收治的120例乳腺癌患者作为研究对象。所有患者均行多参数MRI检查,并通过病理分型分析,记录多参数MRI特征。结果120例患者中,非浸润性肿瘤20例,导管内肿瘤25例,浸润性导管肿瘤62例,浸润性小叶肿瘤13例。不同病理分型乳腺癌患者的MRI形态与边缘情况比较,差异无统计学意义(P>0.05)。导管内肿瘤的MRI-TICⅢ型+Ⅳ型比例都明显高于非浸润性肿瘤、浸润性导管肿瘤与浸润性小叶肿瘤,差异具有统计学意义(P<0.05);以病理诊断结果为金标准,DCE-MRI诊断的灵敏度为93.1%,特异度为54.5%,整体准确度为82.5%。结论多参数MRI技术对不同病理分型乳腺癌的诊断均具有参考价值,值得临床推广应用。

Objective To analyze the diagnostic value of multi-parameter magnetic resonance imaging(MRI)in different types of breast cancer.Methods A total of 120 breast cancer patients admitted to Traditional Chinese Medicine Hospital of Xinjiang Uygur Autonomous Region(Affiliated Hospital of traditional Chinese Medicine of Xinjiang Medical University)from December 2016 to December 2018 were selected as the research subjects.All patients underwent multi-parameter MRI examination,and multi-parameter MRI characteristics were recorded by pathological typing analysis.Results Among the 120 patients,there were 20 cases of non-invasive tumor,25 cases of intraductal tumor,62 cases of invasive ductal tumor,and 12 cases of invasive lobular tumor.There was no statistically significant difference in MRI morphology and marginal condition between patients with different breast cancer types(P>0.05).The proportion of MRI-TICⅢ+Ⅳin patients with intraductal carcinoma was significantly higher than that of non-invasive tumor,invasive ductal carcinoma and invasive lobular carcinoma(P<0.05).Take pathological diagnosis as the gold standard,the sensitivity,specificity and overall accuracy of DCE-MRI were 93.1%,54.5%and 82.5%,respectively.Conclusions Multi-parameter MRI technique has reference value in the diagnosis of different pathological types of breast cancer and is worthy of clinical promotion and application.
