IL-6、D-二聚体及CRP联合检测在未足月胎膜早破高危孕妇早期预测中的应用价值

Clinical value of combined detection of IL-6,D-dimer and CRP in early prediction of high-risk pregnant women with premature rupture of membranes

目的分析白介素-6(IL-6)、D-二聚体及C反应蛋白(CRP)联合检测在未足月胎膜早破高危孕妇早期预测中的应用价值。方法将2018年1月至12月合肥市第三人民医院妇产科诊治的60例未足月胎膜早破高危孕妇设为高危组,30例未足月胎膜早破孕妇设为早破组,30例未破膜正常妊娠临产孕妇设为对照组。采集三组外周血,对IL-6、D-二聚体、CRP水平进行检测和比较,采用Pearson相关性检验对以上指标的相关性进行分析,绘制受试者操作特征(ROC)曲线,分析三者联合预测未足月胎膜早破发生的价值。结果三组IL-6、D-二聚体、CRP水平比较,高危组>早破组>对照组,差异具有统计学意义(P<0.05)。Pearson相关性检验结果显示,IL-6、D-二聚体、CRP水平与未足月胎膜早破均呈正相关(P<0.05)。IL-6、D-二聚体、CRP水平预测未足月胎膜早破发生的cut-off值分别为8.2ng/L、4.1mg/L、5.2mg/L,三者联合预测未足月胎膜早破发生的灵敏度、特异度、曲线下面积(AUC)均分别高于单独预测。结论IL-6、D-二聚体、CRP与未足月胎膜早破的发生呈正相关,在未足月胎膜早破高危孕妇早期预测中联合检测IL-6、D-二聚体及CRP诊断效能更高。

Objective To analyze the clinical value of combined detection of interleukin 6(IL-6),D-dimer and C-reaction protein(CRP)in early prediction of high-risk pregnant women with premature rupture of membranes.Methods 60 cases at high risk of premature rupture of membranes(PRM)in the Department of Obstetrics and Gynecology in Hefei Third People′s Hospital from January to December 2018 were selected as high-risk group,30 pregnant women at low risk of PRM were assigned to the PRM group,and 30 pregnant women of normal delivery without PMS were assigned to the control group.The peripheral blood of the three groups was collected to detect and compare the levels of IL-6,D-dimer and CRP.Pearson correlation test was used to analyze the correlation of the above indexes,and the receiver operating characteristic(ROC)curve was drawn to analyze the combined value of the three factors in predicting the occurrence of premature rupture of membranes.Results The levels of IL-6,D-dimer and CRP in the three groups were statistically significantly different,with a result of the high-risk group>PRM group>control group(P<0.05).Pearson correlation test results showed that IL-6,D-dimer,CRP levels were positively correlated with preterm PRM(P<0.05).The Cut-off values of IL-6,D-dimer and CRP for predicting PRM were 8.2ng/L,4.1mg/L and 5.2mg/L,respectively.The sensitivity,specificity and AUC of the combination of IL-6,D-dimer and CRP for predicting PRM were higher than those predictions with a single indicator.Conclusions IL-6,D-dimer and CRP are positively correlated with the occurrence of PRM,and the combined detection of IL-6,D-dimer and CRP is more effective in the early prediction of pregnant women at high risk of PRM.
