AIDET标准沟通模式联合导乐分娩在初产妇分娩中的应用效果

The effect of AIDET standard communication template combined with doula delivery in primipara delivery

目的探究AIDET标准沟通模式联合导乐分娩在初产妇分娩中的应用效果。方法选取2018年1月至2019年1月无锡市第九人民医院收治的96例初产妇作为研究对象。随机分为观察组(n=48)和对照组(n=48)。对照组给予常规导乐分娩,观察组给予AIDET标准沟通模式联合导乐分娩。采用焦虑自评量表(SAS)对不同时间段初产妇的焦虑状况进行评价,采用视觉模拟评分法(VAS)对不同时间段疼痛程度进行评价,观察并记录初产妇分娩结局。结果干预后,观察组在宫口开至3cm时、产后2h的SAS评分均低于对照组,差异具有统计学意义(P<0.05)。干预后观察组在宫口开至3cm时、宫口全开时的VAS评分均低于对照组,差异具有统计学意义(P<0.05)。观察组干预后自然分娩(无会阴侧切)率高于对照组,剖宫产率低于对照组,差异具有统计学意义(P<0.05);两组新生儿1min和5min的Apgar评分比较,差异无统计学意义(P>0.05)。结论AIDET标准沟通模式联合导乐分娩应用于初产妇分娩过程中,可有效降低其焦虑情绪评分,减轻其分娩过程中不同阶段的疼痛程度,并且有效改善初产妇与新生儿的结局。

Objective To explore the effect of AIDET standard communication template combined with doula delivery in primipara delivery.Methods 96 primiparas treated in Wuxi Ninth People′s Hospital from January 2018 to January 2019 were selected as the research subjects,and randomly divided into observation group(n=48)and control group(n=48).The control group was given conventional doula delivery,and the observation group combined with AIDET standard communication template.Anxiety and pain at different stages of delivery were respectively evaluated by self-rating anxiety scale(SAS)and visual analogue scale(VAS).The childbirth outcome of primiparas was observed and recorded.Results After intervention,the SAS scores of the observation group were lower than those of the control group at 3cm uterine opening and 2h postpartum,with statistical significance(P<0.05).The VAS scores of the observation group were lower than those of the control group when the uterine opening was 3cm and fully open,with statistical significance(P<0.05).The rate of natural delivery(no lateral perineal resection)in the observation group was higher than that in the control group,while the rate of cesarean section was lower than that in the control group,with statistical significance(P<0.05).There was no significant difference in Apgar scores between the two groups at 1min and 5min(P>0.05).Conclusions AIDET standard communication template combined with doula delivery can effectively reduce the anxiety score of primiparas in the process of delivery,relieve their pain at different stages of delivery,and effectively improve the outcome of primiparas and neonates.
