孕中期血清CXCL12、CX3CL1表达与子痫前期的相关性研究 被引量:1

摘要 目的检测孕中期子痫前期(PE)患者血清中趋化因子CXCL12、CX3CL1的表达水平,探讨两者在PE中的临床意义及相关性。方法选取2017年3月至2019年3月广东医科大学附属第三医院(佛山市顺德区龙江医院)收治的60例确诊并住院的PE患者(观察组)以及54例正常孕中期孕妇(对照组)作为研究对象。采用酶联免疫吸附试验(ELISA)法检测血清中CXCL12、CX3CL1表达水平;分析PE患者血清中CXCL12、CX3CL1表达与临床病理特征的关系;分析PE患者血清中CXCL12与CX3CL1表达的相关性;ROC曲线预测CXCL12与CX3CL1对PE的诊断价值。结果观察组血清中CXCL12表达明显低于对照组,CX3CL1表达水平明显高于对照组,差异具有统计学意义(P<0.05);观察组血清CXCL12、CX3CL1表达水平与PE严重程度、是否发生宫内生长受限、早产儿有关(P<0.05);观察组血清CXCL12与CX3CL1表达呈负相关(r=-0.535,P<0.05);ROC曲线显示,CXCL12单独检测的AUC高于CX3CL1(Z=21.250,P=0.000),两者联合检测的AUC高于CXCL12、CX3CL1单独检测(Z=16.000、25.250,P=0.000)。结论孕中期PE患者血清中CXCL12呈低表达,CX3CL1呈高表达,两者负相关且与PE严重程度、宫内生长受限、早产儿有关,两者联合检测对PE具有一定诊断价值。 Objective To detect the expression levels of CXCL12 and CX3CL1 in the serum of patients with preeclampsia(PE)in the second trimester of pregnancy,and to explore the clinical significance and correlation both of them in PE in the second trimester of pregnancy.Methods 60 confirmed and hospitalized PE patients(ob-servation group)and 54 normal pregnant women in the second trimester(control group)admitted to the Third Affili-ated Hospital of Guangdong Medical University(Longjiang Hospital,Shunde District,Foshan City)from March 2017 to March 2019 were selected as the research subjects.The expression levels of CXCL12 and CX3CL1 in serum were detected by enzyme-linked immunosorbent assay(ELISA).The expression of CXCL12 and CX3CL1 in serum of PE patients was analyzed in relation to clinicopathological characteristics.The correlation between CXCL12 and CX3CL1 expression in serum of PE patients was analyzed.The ROC curve predicts the diagnostic value of CXCL12 and CX3CL1 for PE.Results The expression level of CXCL12 in serum of the observation group was significantly lower than that of the control group,and the expression level of CX3CL1 was significantly higher than that of the control group(P<0.05).The expression levels of CXCL12 and CX3CL1 in serum of the observation group were correlated with the severity of PE,intrauterine growth restriction and premature infants(P<0.05).The expression of CXCL12 and CX3CL1 in serum of the observation group was negatively correlated(r=-0.535,P<0.05).The ROC curve showed that the AUC of CXCL12 alone was higher than that of CX3CL1(Z=21.250,P=0.000),and that of CXCL12 and CX3CL1 alone(Z=16.000,25.250,P=0.000).Conclusions The low expression of CX-CL12 and the high expression of CX3CL1 in serum of PE patients during the second trimester were negatively corre-lated with the severity of PE,intrauterine growth restriction and preterm birth.The combined detection of CXCL12 and CX3CL1 has certain diagnostic value for PE.
