前庭自旋转试验在老年中枢血管性急性前庭综合征的诊断价值

Diagnostic Value of Vestibular Autorotation Test in Older Patients with Central Acute Vestibular Syndrome

目的探究前庭自旋转试验在老年中枢血管性急性前庭综合征的诊断价值。方法选取齐齐哈尔医学院附属第三医院2017年2月~2018年12月收治的80例急性前庭综合征患者为病例组,另选取同期接受体检的健康人员80例作为对照组。两组均进行前庭自旋转试验,比较两组各项参数水平。将病例组患者按照临床表现类型的不同分成中枢血管性急性前庭综合征组(44例)和周围性急性前庭综合征组(36例)。比较两组各项前庭自旋转试验参数水平,并分析前庭自旋转试验诊断急性前庭综合征的检出率。结果病例组增益增高伴随减低及增益增高未伴随减低人数占比均高于对照组(P<0.001)。中枢血管性急性前庭综合征患者前庭自旋转试验结果增益增高伴随减低人数占比低于周围性急性前庭综合征患者,而增益增高未伴随减低人数占比高于周围性急性前庭综合征患者(P<0.001)。前庭自旋转试验诊断中枢血管性急性前庭综合征的检出率为97.73%,诊断周围性急性前庭综合征的检出率为88.89%,总检出率为93.75%。结论前庭自旋转试验诊断老年中枢血管性急性前庭综合征的价值较高,值得临床推广应用。

Objective To study the diagnostic value of vestibular autorotation test in older patients with central vascular acute vestibular syndrome.Methods 80 patients with acute vestibular syndrome admitted to the Third Affiliated Hospital of Qiqihar Medical College were selected as the case group from February 2017 to December 2018.In addition,80 cases who underwent physical examination at the same period were selected as the control group.The vestibular autorotation test was performed in both groups.The patients with acute vestibular syndrome were divided into the group with central vascular acute vestibular syndrome(44 cases)and the group with peripheral acute vestibular syndrome(36 cases)according to the clinical manifestations.The parameters of vestibular autorotation test were compared between the two groups.The detection rate of acute vestibular syndrome in vestibular autorotation test was analyzed.Results The proportion of the patients with increased gain accompanied with or without decrease in the case group was higher than that in the control group(P<0.001).In patients with acute vestibular syndrome,the proportion of cases with increased gain and decreased gain was lower,while the proportion of patients with increased gain without decreased gain was higher in patients with central lesion than that in patients with peripheral lesion(P<0.001).The detection rate of central acute vestibular syndrome was 97.73%,the detection rate of peripheral acute vestibular syndrome was 88.89%,and the overall detection rate was 93.75%.Conclusion The vestibular autorotation test shows high value in the diagnosis of central vascular acute vestibular syndrome in older patients.
